目的 研究甲状腺结节(thyroid imaging reporting and data system,TI-RADS)分级与代谢相关指标的关系.方法 非随机选取伊犁哈萨克自治州中医医院于2022年1月—2023年1月期间收治的150例甲状腺结节检出患者为研究对象,经TI-RADS分级分为TI-RADS 3类及以下者(良性组)51例、TI-RADS 4a类(低度恶性组)50例、TI-RADS 4b类及以上组(恶性组)49例,对比3组患者代谢相关指标水平差异,并分析相关性.结果 3组患者总胆固醇、甘油三酯、高密度脂蛋白、HOMA胰岛素抵抗指数、餐后2 h血糖比较,差异无统计学意义(P均>0.05);3组患者低密度脂蛋白(low-density lipoprotein,LDL-C)、糖化血红蛋白(gly-cosylated hemoglobin,HbA1c)、空腹血糖(fasting plasma glucose,FPG)、抗甲状腺球蛋白抗体(anti-thyroglobulin antibodies,TGAb)、体重指数(body mass index,BMI)水平比较,差异有统计学意义(P均<0.05).Pearson相关分析结果显示,甲状腺结节TI-RADS分级与LDL-C、HbA1c、FPG、TGAb水平之间呈现正相关(r=1.334、0.092、0.193、1.294,P均<0.05),甲状腺结节TI-RADS分级BMI水平之间呈现负相关(r=-0.092,P<0.05).结论 代谢相关指标与甲状腺结节的TI-RADS分级之间存在联系,高级别结节患者在BMI、LDL-C、HbA1c、FPG、TGAb指标方面表现出异常.
A Study on the Relationship between Metabolic Related Indicators and TI-RADS Grading of Thyroid Nodules
Objective To study the relationship between the grading of thyroid imaging reporting and data system(TI-RADS)and metabolism related indexes.Methods A total of one hundred and fifty patients with thyroid nodule detec-tion admitted to the Hospital of Traditional Chinese Medicine of Yili Kazak Autonomous Prefecture from January 2022 to January 2023 were selected as the study objects.According to TI-RADS classification,fifty-one cases were classi-fied into TI-RADS class 3 and below(benign group),fifty cases were classified into TI-RADS class 4a(low-grade ma-lignant group),and forty-nine cases were classified into TI-RADS class 4b and above(malignant group).The levels differences of metabolic related indexes among the three groups were compared,and the correlation was analyzed.Re-sults Total cholesterol,triglyceride,high density lipoprotein,HOMA insulin resistance and 2 hours postprandial blood glucose were compared in 3 groups,there was no significant difference(all P>0.05).Low density lipoprotein(LDL-C),glycosylated hemoglobin(HbA1c),fasting plasma glucose(FPG),glycosylated hemoglobin(HbA1c),anti-thyroglobulin antibodies(TGAb)and body mass index(BMI)were compared in 3 groups,and the differences were statistically sig-nificant(all P<0.05).Pearson correlation analysis showed that the TI-RADS grade of thyroid nodules was positively correlated with LDL-C,HbA1c,FPG and TGAb levels(r=1.334,0.092,0.193,1.294,all P<0.05).There was a nega-tive correlation between the BMI level of TI-RADS classification of thyroid nodules(r=-0.092,P<0.05).Conclusion There is a significant correlation between metabolism-related indexes and TI of thyroid nodules-RADS grading were associated,and patients with high-grade nodules showed abnormalities in BMI,LDL-C,HbA1c,FPG,and TGAb indi-ces.
