浆膜腔积液脱落细胞学及肿瘤标志物检查在恶性肿瘤浆膜腔积液诊断中的应用价值

摘要
目的 探究在恶性肿瘤所致浆膜腔积液的诊断中,脱落细胞学检查与肿瘤标志物检测的应用价值.方法 非随机选取 2023年 1月—2023年 12月微山县人民医院医学检验科收治的 280例疑似恶性肿瘤的浆膜腔积液患者为研究对象.以病理诊断结果为金标准,分析浆膜腔积液的脱落细胞学检查和肿瘤标志物检测的诊断效能,通过Kappa值进行一致性分析,以评价其诊断价值.结果 经病理学检查确诊恶性肿瘤患者 95例,阴性 185例.一致性分析显示,浆膜腔积液脱落细胞学检查与肿瘤标志物检测的联合应用与病理检查结果具有较高的一致性(Kappa=0.976).联合诊断的灵敏度、特异度、准确度、阳性预测值、阴性预测值均高于单独使用脱落细胞学检查或肿瘤标志物检测,差异有统计学意义(P均<0.05).结论 在恶性肿瘤导致的浆膜腔积液诊断中,脱落细胞学检查与肿瘤标志物检测均显示出一定的应用价值,且二者联合使用可以增强诊断的准确性.
Abstract
Objective To explore the application value of exfoliative cytology and tumor marker detection in the diag-nosis of serous effusion caused by malignant tumors.Methods 280 patients with serous cavity effusion suspected of malignant tumors admitted to the Department of Medical Laboratory of Weishan County People's Hospital from Janu-ary 2023 to December 2023 were non-randomly selected as study objects.Based on the pathological diagnosis results as the gold standard,the diagnostic efficacy of exhalation cytology and tumor marker detection of serosal effusion was analyzed,and the diagnostic value was evaluated by consistency analysis of kappa values.Results 95 patients with ma-lignant tumors were confirmed by pathological examination,and 185 cases were negative.Consistency analysis showed that the combined application of serous cavity effusion exfoliative cytology and tumor marker detection had a high con-sistency with pathological examination results(Kappa=0.976).The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of combined diagnosis were higher than those of exfoliative cytology or tumor markers alone,and the differences were statistically significant(all P<0.05).Conclusion In the diagnosis of serous ef-fusion caused by malignant tumors,both exfoliative cytology and tumor marker detection show certain application value,and the combined use of the two can enhance the accuracy of diagnosis.
