目的 将括约肌间瘘管结扎(ligation of intersphincteric fistulatract,LIFT)与瘘管隧道式切除术联合用于肛瘘中,探究其治疗效果.方法 回顾性选取2021年6月—2023年9月浙江省人民医院毕节医院收治的87例肛瘘患者的临床资料,以手术方案分组,对照组41例单纯行LIFT,观察组46例行LIFT联合瘘管隧道式切除术.比较两组术后恢复、疼痛程度、水肿程度、肛门溢液发生率、肛门评分各指标差异.结果 观察组术后首次排便、创面愈合及住院时间更短,差异有统计学意义(P均<0.05).观察组患者术后3、7 d的疼痛程度、肛周水肿程度、肛门溢液发生率均低于对照组,差异有统计学意义(P均<0.05).两组术前肛门功能评分对比,差异无统计学意义(P>0.05).观察组患者术后肛门功能评分为(1.62±0.81)分,低于对照组的(3.01±1.07)分,差异有统计学意义(t=6.578,P<0.05).结论 对肛瘘患者而言,LIFT联合瘘管隧道式切除术可获得更理想的治疗效果,促使术后创面更快恢复,有效减轻疼痛及水肿程度,减少肛门溢液发生,改善患者肛门功能,值得临床借鉴.
Effect Analysis of Intersphincter Fistula Ligation Combined with Fistula Tunnel Resection in the Treatment of Anal Fistula
Objective To explore the therapeutic effect of ligation of intersphincteric fistulatract(LIFT)combined with tunnel resection of fistulatract in anal fistulas.Methods The clinical data of eighty-seven patients with anal fistula ad-mitted to Bijie Hospital of Zhejiang Provincial People's Hospital from June 2021 to September 2023 were retrospec-tively selected.The patients were grouped by surgical plan.Forty-one patients in the control group received LIFT alone,and forty-six patients in the observation group received LIFT combined with tunnel resection of fistula.The postoperative recovery,pain degree,edema degree,incidence of anal discharge and anal score were compared between the two groups.Results The first postoperative defecation,wound healing and hospital stay in the observation group were shorter,and the differences were statistically significant(all P<0.05).The degree of pain,perianal edema and in-cidence of anal discharge in the observation group were lower than those in the control group at 3 and 7 days after op-eration,the differences were statistically significant(all P<0.05).There was no statistically significant difference in preoperative anal function scores between the two groups(P>0.05).The postoperative anal function score of observa-tion group was 1.62±0.81,which was lower than 3.01±1.07 of control group,and the difference was statistically signifi-cant(t=6.578,P<0.05).Conclusion For patients with anal fistula,LIFT combined with fistula tunnel resection can ob-tain more ideal treatment effect,promote faster postoperative wound recovery,effectively reduce pain and edema,re-duce the occurrence of anal discharge,and improve the anal function of patients,which is worthy of clinical reference.
