摘要
目的:探究细致管理联合延伸护理对慢性心力衰竭(CHF)患者心功能、运动耐力、心理状态、自护能力和生活质量的影响.方法:选取济宁医学院附属医院2023年1月至6月收治的121例CHF患者,按照随机数字表法分为干预组(n=61)和对照组(n=60),对照组实施常规护理干预,干预组实施细致管理联合延伸护理,两组均干预6个月.对比两组患者护理前后心功能情况、运动耐力、心理状态、自护能力与生活质量.结果:与对照组比较,干预组护理后左室收缩末期内径(LVESd)[(44.87±3.02)mm比(40.10±2.14)mm]、左室舒张末期内径(LVEDd)[(51.55±3.50)mm 比(47.32±3.02)mm]、心率[(85.29±2.97)次/min 比(81.50±5.05)次/min]、汉密尔顿焦虑量表(HAMA)[(18.57±1.43)分比(11.88±1.07)分]、汉密尔顿抑郁量表(HAMD)[(18.46±1.59)分比(13.26±1.10)分]、汉化版欧洲心力衰竭自我护理能力量表(EHFScBS-C)评分[(17.88±1.62)分比(13.55±1.87)分]、明尼苏达心力衰竭生活质量问卷(MLHFQ)其他领域[(14.10±1.55)分比(6.97±1.39)分]、躯体领域[(19.47±2.48)分比(11.17±2.27)分]和情感领域[(9.31±1.69)分比(4.90±1.66)分]维度分、不良事件发生率(23.33％比8.33％)显著降低;6min步行距离(6MWD)[(434.93±4.31)m比(481.01±4.59)m]显著增加(P＜0.05或＜0.01).结论:细致管理联合延伸护理能够显著改善CHF患者心功能,提高其运动耐力和自护能力,缓解负面情绪,改善生活质量,值得在临床中推广应用.
Abstract
Objective:To explore the effects of meticulous management combined with extended nursing on cardiac function,exercise endurance,mental state,self-care ability and quality of life in patients with chronic heart failure(CHF).Methods:A total of 121 CHF patients admitted to Affiliated Hospital of Jining Medical College between January and June 2023 were included,divided into intervention group(n=61)and control group(n=60)according to random number table method.The control group received routine nursing intervention,and the intervention group received meticulous nursing combined with extended nursing,and both groups received intervention for 6 months.Cardiac function,exercise endurance,mental state,self-care ability and quality of life were compared be-tween the two groups before and after nursing.Results:Compared with those in control group after nursing,pa-tients in intervention group had significant lower left ventricular end systolic diameter(LVESd)[(44.87±3.02)mm vs.(40.10±2.14)mm],left ventricular end-diastolic diameter(LVEDd)[(51.55±3.50)mm vs.(47.32±3.02)mm]and heart rate[(85.29±2.97)beats/min vs.(81.50±5.05)beats/min],scores of Hamilton anxiety scale(HAMA)[(18.57±1.43)points vs.(11.88±1.07)points],Hamilton depression scale(HAMD)[(18.46±1.59)points vs.(13.26±1.10)points],the European Heart Failure Self-care Behaviour Scale for Caregivers(EHFScBS-C)[(17.88±1.62)points vs.(13.55±1.87)points],dimensional scores of other fields[(14.10±1.55)points vs.(6.97±1.39)points],somatic domain[(19.47±2.48)points vs.(11.17±2.27)points]and af-fective domain[(9.31±1.69)points vs.(4.90±1.66)points]of Minnesota living with heart failure questionnaire(MLHFQ),incidence of adverse events(23.33％vs.8.33％)and significant higher 6min walking distance(6MWD)[(434.93±4.31)m vs.(481.01±4.59)m](P＜0.05 or＜0.01).Conclusion:Meticulous management combined with extended nursing can significantly improve the cardiac function,exercise endurance and self-care a-bility,alleviate negative emotions,and improve quality of life in CHF patients,which is worthy of clinical promo-tion and application.
