Clinical effect of total hip arthroplasty and hemiarthroplasty in the treatment of femoral neck fractures in middle-aged and elderly patients in Xizang
Objective To investigate the short-term clinical effect of total hip arthroplasty(THA)and hemiarthroplas-ty(HHR)in the treatment of femoral neck fracture.Methods A randomized cohort study was conducted.A total of 30 patients were included in the study sample,all of whom underwent joint replacement in the Department of Ortho-paedics,People's Hospital of Xizang Autonomous Region from January 2021 to December 2022.The patients were divided into two groups by bicolor ball method.The red ball represented the total hip group and the yellow ball repre-sented the hemihip group.In order to ensure the accuracy of the data,30 patients were equally divided,with 15 cases in each group.The patients were enrolled according to the lottery results.The two treatment methods,as group names,were studied for the short-term clinical effectss.The patients were followed up for operation time,hospital stay,intraoperative blood loss,postoperative rehabilitation time,and whether the patients were able to go up and down stairs alternately,and were able to wear their lower body independently,including putting on and taking off shoes and socks,and could perform special functional activities(such as cycling and swimming),as well as the hip Harris score,SF-36 score and postoperative complication rate on the 7th postoperative day,and 1st month,2nd month and 3rd month after surgery.Results Surgery-related indicators showed that the hemihip group had shorter operation time,hospital stay and less intraoperative bleeding(P<0.05),but there was no significant difference in recovery time(P>0.05).According to the data of Harris scale,the joint function performance of the total hip group was more ideal,and the scores of all dimensions were higher than those of the hemihip group(P<0.05).According to the SF-36 data,the quality of life of the total hip group was higher,and the scores of all dimensions were higher than those of the hemihip group(P<0.05).Postoperative complications occurred in both groups,including 1 case in the total hip group and 4 cases in the hemihip group,and there was a significant difference in the data comparison(6.67%vs 26.67%)(P<0.05).Conclusion Total hip arthroplasty for the treatment of femoral neck fracture group had a higher quality of life and fewer complications,but compared with hemiarthroplasty,the operation time was longer and the amount of blood loss is more.Both treatments had their own advantages,of which hemiarthroplasty was more suit-able for elderly patients.Therefore,the surgical method should be selected according to the patient's condition in clini-cal practice.
