摘要
目的:基于数据挖掘分析曾永蕾治疗肺癌术后用药规律.方法:收集曾永蕾2021年1月—2024年6月门诊治疗肺癌术后患者的医案,利用古今医案云平台V2.3.5对中药处方进行频次、四气、五味、归经和功效统计分析,以及中药关联分析、聚类分析、复杂网络分析等.结果:经筛选后共收集179则医案,处方涉及159味中药,累计用药3 236次.高频中药包括浙贝母、黄芪、陈皮、苦杏仁、干鱼腥草等;药性以平、温、寒、微寒、微温为主,药味以甘、苦、辛为主,归经以肺、脾、肝、心经为主;功效以清热解毒散结、润肠通便利水、燥湿化痰止咳、补气养血益阴等为主.核心药对为"苦杏仁-浙贝母""太子参-黄芪""姜半夏-陈皮"等;聚类分析将前37味高频中药聚为7类;复杂网络分析出的核心组方包含14味中药.结论:曾永蕾治疗肺癌术后患者以扶正补虚为本,清热解毒、化痰祛瘀等为标,兼顾纳寐及二便,少用刺激攻伐之药,处方药味精简,剂量适中.
Abstract
Objective:To analyze the medication rules of ZENG Yonglei in the treatment of post-operative lung cancer based on data mining.Methods:Medical records of post-operative lung cancer patients treated by ZENG Yonglei from January 2021 to June 2024 were collected.The Chinese herbal prescriptions were analyzed for frequency,four natures,five flavors,channel entry,and effects via the Ancient and Modern Medical Records Cloud Platform V2.3.5.Additionally,association analysis,cluster analysis,and complex network analysis of Chinese herbs were conducted.Results:After screening,179 medical records were collected,involving 159 Chinese herbs with a total of 3 236 times of use.High-frequency herbs included Fritillariae Thunbergii Bulbus,Astragali Radix,Citri Reticulatae Pericarpium,Armeniacae Semen Amarum,and Houttuyniae Herba.The main natures were neutral,warm,cold,mild cold,and mild warm;the main flavors were sweet,bitter,and acrid;the main channel entries were lung,spleen,liver,and heart.The main effects included clearing heat,resolving toxins and dissipating masses,moistening the intestines to promote defecation and promote urination,drying dampness,dissolving phlegm and relieving cough,and supplementing qi,nourishing blood and boosting yin.Core herbal pairs included Armeniacae Semen Amarum-Fritilariae Thunbergii Bulbus,Pseudostellariae Radix-Astragali Radix,and Pinelliae Rhizoma Praeparatum Cum Zingibere et Alumine-Citri Reticulatae Pericarpium.Cluster analysis grouped the top 37 high-frequency herbs into 7 categories.The core prescription identified through complex network analysis contained 14 herbs.Conclusion:ZENG Yonglei treatment of post-operative lung cancer patients focuses on reinforcing healthy qi and supplementing deficiency as the fundamental root,while clearing heat and resolving toxins,dissolving phlegm and removing stasis are the branch.Attention is also paid on appetite and sleep,urination and bowel functions,with minimal use of stimulating and aggressive herbs.The prescriptions are concise with moderate dosages.
