Diagnostic efficacy of intratumoral and peritumoral blood flow signals under color Doppler ultrasound in differentiating primary liver cancer from liver metastases
Objective To analyze the efficiency of intratumoral and peritumoral blood flow signals under color Doppler ultra-sound in the differential diagnosis of primary liver cancer and liver metastases.Methods 95 cases of liver cancer confirmed by sur-gery and pathology were retrospectively analyzed in our hospital(including 46 cases of primary liver cancer and 49 cases of liver metastasis).A case.All cases were examined by color Doppler ultrasound before operation,and the intensity of intra-and peri-tu-mor blood flow signal was recorded and compared between patients with primary liver cancer and patients with liver metastases.With the pathological results of surgery as the"gold standard",the ultrasound results were compared with the pathological results af-ter surgery,and the nature of the ultrasound results(true positive,true negative,false positive,false negative)was judged case by case.Results Intratumoral blood flow signal:the rate of grade 0 in patients with primary liver cancer was significantly lower than that in patients with liver metastasis(P<0.05),and the rate of grade Ⅱ was significantly higher than that in patients with liver metas-tasis(P<0.05).Peritumoral blood flow signal:the grade 0 rate of primary liver cancer patients was significantly lower than that of liver metastasis patients(P<0.05),and the grade Ⅲ rate was significantly higher than that of liver metastasis patients(P<0.05).The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of color ultrasound were 91.30%(42/46),87.76%(43/49),89.47%(85/95),87.50%(42/48)and 91.49%(43/47)respectively.The Kappa value was 0.790.Conclusion There are significant differences in intra-and peri-tumor blood flow signals between primary liver cancer and liver metastases under color ultrasound,which can provide evidence for clinical differentiation.
