Analysis of the diagnostic efficacy of body composition indicators for abnormal glucose and lipid metabolism in middle-aged people aged 45 years and above in Kangding City in 2021
Objective To analyze the current situation of composition and glucose and lipid metabolism in the middle-aged and elderly population in Kangding City,Sichuan Province,to study the diagnostic efficacy of human composition indicators on glucose and lipid metabolism abnormalities,and to provide reference basis for the prevention and control of chronic diseases in the middle-aged and elderly population.Methods From September to October 2021,the convenient sampling method was used to select middle-aged and elderly residents in Kangding City of Sichuan Province for a questionnaire survey,body composition tests and laboratory tests.T-test was applied to compare the differences in group composition between people with abnormal glucose and lipid metabolism and normal people.Spearman correlation analysis was used to analyze the correlation between glucose and lipid metabolism indexes and body composition indexes.The receiver operating characteristic(ROC)curve was used to study the diagnostic efficacy of body composition indexes for abnormal glucose and lipid metabolism.And the difference was considered statistically significant with P<0.05.Results A total of 1056 cases were included in the survey,including 722 males and 334 females,with an average age of(56.42±6.85)years.Impaired glucose regulation accounted for 14.0%,abnormal lipid metabolism accounted for 51.3%,and abnormal metabolism of both glucose and lipid accounted for 10.0%.The skeletal muscle mass index(SMI),body mass index(BMI),percentage of body fat(PBF),visceral fat area(VFA)and waist-to-hip ratio(WHR)in impaired blood glucose regulation group were higher than those in normal group(all P<0.05).SMI,skeletal muscle mass(SMM),lean tissue mass(SLM),BMI,PBF,VFA and WHR were higher in the abnormal lipid metabolism group and in the group with both abnormal glucose and lipid metabolism than those of normal group(P<0.05).BMI,PBF,VFA and WHR were positively correlated with fasting glucose(all P<0.05).PBF,VFA and WHR were positively correlated with total cholesterol(TC),triglycerides(TG)and low-density lipoprotein cholesterol(LDL-C)(all P<0.05).SMI,SLM and SMM were negatively correlated with TC and high-density lipoprotein cholesterol(HDL-C)(all P<0.05).The body composition indexes with correlation with glucose and lipid were selected and analyzed by ROC curve,and BMI,PBF,VFA and WHR could predict impaired glucose regulation,among which WHR had the highest diagnostic efficacy with an ideal cut-off point of 0.88(P<0.05).SMI,SLM,SMM,BMI,PBF,VFA and WHR could predict the risk of dyslipidemia and abnormalities in both glucose and lipid metabolism,and both had the highest diagnostic efficacy,with ideal cut-off points of 24.65 kg/m2 and 24.45 kg/m2,respectively(P<0.05).Conclusions Abnormalities in glucose and lipid metabolism exist in the middle-aged and elderly population in Kangding City,Sichuan Province,with the highest rate of dyslipidemia.WHR and BMI of body composition are good indexes for predicting abnormal glucose and lipid metabolism.
