Changes of Serum miR-221 and miR-146a expression in AIDS patients and their relationship with HIV load and prognosis
Objective The changes of serum miR-221 and miR-146a expression in AIDS patients and their relationship with human immunodeficiency virus(HIV)load and prognosis.Methods Total 104 AIDS patients who visited Qionglai Medical Center Hospital from January 2020 to January 2023 were selected as the research group.According to the treatment results after 6 months,the patients were divided into a good prognosis group and a poor prognosis group;Divide into low dose group,medium dose group,and high dose group based on HIV load.106 healthy volunteers from the same period were selected as the control group.Clinical data of all research subjects and pathological data of AIDS patients were collected for comparative analysis;Fluorescence quantitative PCR was used to detect the expression levels of miR-221 and miR-146a in serum;Multivariate logistic regression was used to analysis the influencing factors on the prognosis of AIDS patients;Receiver Operating Characteristic(ROC)curve analysis on the predictive efficacy of miR-221 and miR-146a levels on adverse prognosis in AIDS patients,Z-test was used to compare the area under the curve(AUC);The difference P<0.05 was considerded statistically significant.Results There was no difference in age and gender between the control group and the study group(P>0.05),it had indicating comparability;The expression level of miR-221 in the study group was higher than that in the control group(t=9.861,P<0.001),and the expression level of miR-146a was lower than that in the control group(t=9.118,P<0.001).Further comparative results showed that the expression levels of miR-221 and miR-146a in AIDS patients were not related to age,gender,and transmission route(P>0.05),but those were closely related to genital herpes,lymphoma,and HIV load(miR-221:t=4.548,t=5.734,t=19.471,P<0.001;miR-146a:t=6.532,t=3.116,t=11.441,P<0.05).Compared with the group with good prognosis,the expression level of serum miR-221 was up-regulated(t=8.689,P<0.001)and the expression level of miR-146a was decreased(t=8.598,P<0.001)in the group with poor prognosis.Genital herpes,lymphoma,high HIV load,and high expression of miR-221 were all risk factors for poor prognosis in AIDS patients(P<0.05),while high expression of miR-146a was a protective factor for poor prognosis in AIDS patients(P<0.05).Through ROC curve analysis,the AUC of miR-221,miR-146a,and their combination in predicting adverse prognosis in AIDS patients were 0.928,0.915,and 0.983,respectively.The combination of that two was better than the individual prediction of miR-221 and miR-146a(Zcombination-miR-22i=2.223,Zcombination-miR-146a=2.353,P=0.026,0.019).Conclusions The levels of miR-221 and miR-146a have an influence on the occurrence of AIDS,which is closely related to its pathological features and HIV load.Both of them have the efficacy of predicting the prognosis of AIDS patients,and the combined diagnosis value of them is higher.
