Analysis of deaths in HIV/AIDS patients aged 15 years and above reported in Shehong City from 2005 to 2022
Objective To analyze the death situation and influencing factors of HIV/AIDS cases in Shehong City from 2005 to 2022,and to provide evidence for the control of AIDS death.Methods Through the Chinese Disease Prevention and Control Information System,the relevant information of HIV/AIDS deaths over 15 years old in Shehong city from 2005 to 2022 was collected to describe the characteristics of death and the trend of death ratio over time.Cox proportional risk model was used to analyze the influencing factors of survival time,and the difference P<0.05 was considered statistically significant.Results As of December 31,2022,a total of 464 HIV/AIDS deaths aged 15 years and above were reported in Shehong City,with an average age of confirmation of(57.69±16.40)years and an average age of death of(59.55±15.93)years,87.07%of them were males,63.58%were HIV patients at the time of death,90.30%had a junior high school education,and 65.30%were farmers and migrant workers.96.55%of those patients were detected by medical institutions,67.24%of patients with initial CD4 test value less than 200/μL,67.24%of patients over 50 years old died,43.53%of patients did not receive antiviral treatment.Among the main causes of death,non-AIDS-related deaths accounted for 84.70%,and the proportion of deaths decreased from 25.00%to 3.65%during 2005-2022,and the average survival time increased from 0.00 months to 27.67 months.Cox regression model analysis showed that male(HR=1.71),AIDS(HR=1.68),farmer and migrant worker occupation(HR=1.60),heterosexual transmission(HR=1.91),medical institution detection(HR=2.17),and age above 50 years old at the time of confirmation had a higher risk of death.The risk of death was lower with the first CD4 test of more than 200/μl(HR=0.41)and antiviral treatment(HR=0.09).Conclusions In recent years,the death rate of HIV patients in Shehong City has increased year by year,and the death rate has decreased year by year.It is necessary to continue to promote the full implementation of the strategy of"early detection,early intervention and follow-up in place"to discover and treat HIV/AIDS patients as early as possible.
