Timeliness of antiretroviral therapy initiation and its influencing factors among newly reported HIV/AIDS cases in Dali Prefecture from 2018 to 2022
Objective To understand the timeliness of antiretroviral therapy(ART)initiation in newly reported HIV/AIDS(human immunodeficiency virus/acquired immunodeficiency syndrome)cases in Dali Prefecture in recent years,and to analyze its influencing factors.Methods Follow-up and ART data of newly reported HIV/AIDS cases from 2018 to 2022 in Dali Prefecture were downloaded from the China Disease Prevention and Control Information System.Descriptive methods,Chi-square test and multivariate Logistic regression were used to analyze the timeliness of ART and its influencing factors.P<0.05 was considered to be statistically significant.Results ART was timely initiated in 60.44%(327/541)of the studied patients in Dali Prefecture from 2018 to 2022.The median time to timely initiation of ART was 10 days.There were statistically significant differences in the timely rate of receiving treatment in the 5 years(x2=22.308,P<0.001).The highest timely rate was in 2020.Multivariate Logistic regression analysis showed that the rate of timely treatment in female patients was higher than that in male patients(odds ratio OR=1.784,95%confidence interval CI:1.061-2.999).The timely rate of treatment was relatively lower under the following conditions:non-local referrals(OR=0.387,95%CI:0.196-0.762),divorced or widowed(OR=0.563,95%CI:0.344-0.950),retired(OR=0.31,95%CI:0.138-0.696),hospital-based diagnosis(OR=0.332,95%CI:0.173-0.639),diagnosis from other institutions(OR=0.448,95%CI:0.221-0.905),initial CD4 cell count ≥500 cells/pl(OR=0.445,95%CI:0.220-0.899),lack of CD4 cell testing(OR=0.008,95%CI:0.001-0.062)and initial CD4 cell testing>14 days(OR=0.105,95%CI:0.062-0.181).Conclusions The timeliness of antiviral treatment in Dali Prefecture was affected by reporting region,gender,marriage,population classification,sample source,first CD4 cell count,first CD4 cell detection time and other factors.Intervention can be conducted according to the above influencing factors to improve the timely rate of antiviral treatment.
