2023年凉山彝族自治州中小学生超重肥胖现状及影响因素分析

目的 对2023年凉山彝族自治州中小学生超重肥胖现状及影响因素进行分析,为彝族地区中小学生超重肥胖防控提供依据.方法 依托"2023年全国学生常见病和健康影响因素监测与干预项目",采取分层随机整群抽样方法,抽取凉山州17个县(市)中小学生进行体格检查及饮食运动情况问卷调查.采用SPSS 23.0软件进行x2检验、多因素Logistic回归分析,检验水准α=0.05.结果 2023年调查凉山州中小学生30412人,超重肥胖检出率18.0%,超重检出率11.7%,肥胖检出率6.3%.超重肥胖检出率男生(18.9%)高于女生(17.1%)(x2=15.960,P<0.001),城区学生(22.4%)高于郊县学生(16.9%)(x2=98.693,P<0.001).x2检验结果显示,正常和超重肥胖学生是否住校、过去1周新鲜水果频次、新鲜水果种类、吃蔬菜频次、蔬菜种类,喝牛奶、酸奶、豆浆或豆奶频率,吃早餐频率,过去1周60 min以上中高强度运动天数、周末节假日中高强度运动60min以上天数等差异有统计学意义(P值均小于0.05).多因素Logistic回归分析结果显示,超重肥胖风险郊县学生低于城区学生(OR=0.780,95%CI:0.708~0.860),女生低于男生(OR=0.907,95%CI:0.839~0.981),高中生高于小学生(OR=1.153,95%CI:1.015~1.310),不住校学生高于住校学生(OR=1.312,95%CI:1.189~1.449),过去 1周60min以上中高强度运动天数4~7d学生高于≤1 d学生(OR=1.111,95%CI:1.003~1.230).结论 凉山州中小学生超重肥胖检出率增高,超重肥胖与人口学特征、饮食及运动行为有关,应有针对性的采取干预措施,以减少彝族地区中小学生超重肥胖的发生.
Analysis on the current situation and influencing factors of overweight and obesity among primary and secondary school students in Liangshan Yi Autonomous Prefecture in 2023
Objective To investigate the current situation and influencing factors of overweight and obesity among primary and secondary school students in Liangshan Yi Autonomous Prefecture in 2023,and to provide a basis for the prevention and control of overweight and obesity among primary and secondary school students in the Yi ethnic area.Methods Based on the 2023 National Student Common Diseases and Health Influencing Factors Surveillance and Intervention Project,stratified random cluster sampling method was used to select primary and secondary school students from 17 counties/cities in Liangshan Prefecture for physical examination,diet and exercise questionnaire survey.Chi-square test and Logistic regression analysis were conducted by SPSS 23.0 software for analysis,and the test level was α=0.05.Results In 2023,a total of 30 412 primary and secondary school students were investigated,and the detection rate of overweight and obesity was 18.0%.The detection rate of overweight and obesity was 11.7%and 6.3%,respectively.The prevalence of overweight and obesity in male students(18.9%)was higher than that in female students(17.1%)(x2=15.960,P<0.001),and that in urban students(22.4%)was higher than that in suburban students(16.9%)(x2=98.693,P<0.001).Chi-square test results showed that there were statistically significant differences in whether the normal and overweight/obese students lived in school,the frequency of fresh fruits in the past week,the number of types of fresh fruit,the frequency of eating vegetables,the number of types of vegetables,the frequency of drinking milk,yogurt,soybean milk or soy milk,the frequency of eating breakfast,the number of days of medium and high intensity exercise of more than 60 minutes in the past week,and the number of days of medium and high intensity exercise of more than 60 minutes in weekends and holidays(all P<0.05).The results of multivariate Logistic regression analysis showed that the risk of overweight and obesity among suburban students was lower than that of urban students(odds ratio OR=0.780,95%confidence interval CI:0.708-0.860),that of female students was lower than that of male students(OR=0.907,95%CI:0.839-0.981),that of high school students was higher than that of elementary school students(OR=1.153,95%CI:1.015-1.310),that of non-resident students was higher than that of resident students(OR=1.312,95%CI:1.189-1.449),and that of students who had engaged in moderate to high intensity exercise for more than 60 minutes in the past week for 4-7 days was higher than that of students who had engaged in exercise less than 1 day(OR=1.111,95%CI:1.003-1.230).Conclusions The prevalence of overweight and obesity among primary and secondary school students in Liangshan Prefecture had increased.Overweight and obesity were related to demographic characteristics,diet and exercise behavior.Targeted intervention measures should be taken to reduce the incidence of overweight and obesity among primary and secondary school students in the Yi ethnic area.
