Analysis on 5 183 cases of cervical high-risk HPV infection in women and influencing factors
Objective To analyze 5 183 women with high-risk human papillomavirus(HPV)infection and the influencing factors,so as to provide reference for cervical cancer prevention,treatment and education.Methods From March,2019 to August,2022,5 183 women who were tested for HPV in the women's insurance clinic of Hangu Sub-center of Tianjin Binhai New Area Maternal and Child Health Family Planning Service Center were selected as the research objects,and the infection rate,infection subtype and age distribution of high-risk HPV were analyzed,and the infection factors of high-risk HPV were summarized by single factor and logistic multiple regression analysis.Results Among the 5 183 patients who attended the clinic,537 cases of HPV high-risk positive infections were detected,with a total infection rate of 10.36%,including 446 cases of single subtype,accounting for 83.05%(446/537)of positive patients,74 cases of 2-stage infection,accounting for 13.78%(74/537),and 17 cases of 3-stage infection or above,accounting for 3.17%(17/537).All 14 high-risk HPV subtypes were detected,and the top 5 in order of detection rate were HPV16(19.18%),HPV52(15.46%),HPV58(14.15%),HPV51(8.75%),and HPV18(8.01%).The difference in high-risk HPV positivity rate between different age groups was not statistically significant(x2=3.382,P=0.336),but HPV16 was the highest in all age groups,especially in the 55+age group(22.73%).Factors such as education level(OR=1.79,95%CI:0.83-4.24,P=0.04),age at first sexual contact(OR=3.24,95%CI:1.41-7.48,P<0.01),number of abortions(OR=2.27,95%CI:1.12-4.48,P=0.02),number of sexual partners(OR=2.72,95%CI:1.12-6.49,P<0.01)and degree of cervical erosion(OR=3.89,95%CI:1.78-8.53,P<0.01)were all risk factors for HPV infection.Conclusion The prevalence of high-risk HPV infection in the region was 10.36%,with HPV 16,52,58,51 and 18 predominating in women over 55 years of age,and that age at first sexual contact,education level,number of abortions,number of sexual partners and degree of cervical erosion may be risk factors for HPV infection.
