810 nm半导体激光与果酸治疗毛周角化症临床疗效分析

目的 探讨810 nm半导体激光与果酸两种治疗方法治疗毛周角化症的疗效分析.方法 选择 2020 年 9月—2022年3月于皮肤科门诊就诊的手臂毛周角化症患者43例,随机分成自身对照组18例,采用810 nm半导体激光治疗左上臂,70%果酸治疗右上臂,联合治疗组25例,采用 810 nm半导体激光及 70%果酸治疗方案交替治疗双上臂,间隔4周治疗一次,共治疗3次,对比分析1个月和6个月后的临床疗效.结果 患者治疗后分别于 1 个月、6 个月复诊,治疗后1个月三组临床总显效率比较差异无统计学意义(P>0.05).治疗后 6 个月三组临床总显效率比较差异有统计学意义(P=0.003),联合治疗组的总显效率均高于果酸治疗和激光治疗(P<0.05 或 0.01).治疗后 1 个月所有受试者的角质层含水量均明显提升,于6个月后再次测试数值有所回落,各组间比较差异有统计学意义(P<0.01),治疗 6个月后,联合治疗组角质层含水量更高(P<0.01).治疗后1个月所有受试者的经皮水分丢失明显减少,于 6 个月后再次测试果酸治疗和激光治疗的数值回升,各组间比较差异有统计学意义(P<0.01),治疗 6 个月后,联合治疗组经皮水分丢失量更低(P<0.01).治疗期间所有患者未见明显不良反应.结论 810 nm半导体激光联合果酸治疗毛周角化症的疗效优于单用激光或果酸治疗,其不良反应少,操作方便,近期疗效好,值得临床推广应用.
Clinical analysis of 810 nm semiconductor laser and fruit acid in the treatment of keratosis pilaris
Objective To investigate the curative effect of 810 nm semiconductor laser and fruit acid skin rejuvenation in the treatment of keratosis pilaris.Methods 43 patients with arm peridermal keratosis who were treated in outpatients in the department of dermatology from September 2020 to March 2022 were randomly divided into self control group(18 cases)and combined treatment group(25 cases).In the control group,left upper arms were treated with 810 nm semiconductor laser,and right upper arms were treated with 70%fruit acid,while in the combined treatment group,both upper arms were treated with 810 nm semiconductor laser and 70%fruit acid alternately.Treatment interval was 4 weeks for a total of 3 times.And then,clinical efficacy after 1 month and 6 months was compared and analyzed.Results After treatment,patients were revisited at 1 month and 6 months,respectively,and there was no statistically significant difference in the overall clinical efficacy among the three groups at 1 month after treatment(P>0.05).After 6 months of treatment,there was statistically significant difference in the overall clinical efficacy among the three groups(P=0.003),and the combined treatment group had a higher overall effica-cy than both fruit acid treatment and laser treatment(P<0.05 or 0.01).After 1 month of treatment,the water content in the stratum corneum of all subjects significantly increased,but after 6 months of retesting,the values decreased slightly,and there was statistically significant difference between the groups(P<0.01).After 6 months of treatment,the combined treatment group had a higher water content in the stratum corneum(P<0.01).After 1 month of treatment,the transcutaneous water loss of all subjects was significantly reduced.After 6 months,the values of fruit acid treatment and laser treatment were tested a-gain,and the values showed a rebound,and the differences between the groups were statistically significant(P<0.01).After 6 months of treatment,the combined treatment group had less transcutaneous water loss(P<0.01).No significant adverse re-actions were found in all patients during treatment.Conclusion The curative effect of 810 nm laser combined with fruit acid in the treatment of peridermal keratosis is better than that of laser or fruit acid alone.It has fewer adverse reactions,convenient operation and good short-term effect,and thus is worthy of clinical application.
