Changes of cytokines IL-6,γ-IFN,IL-10 in EBV infectious mononucleosis in children
Objective To investigate the changes of lymphocyte subsets,cytokines IL-6,γ-IFN and IL-10 in infectious mononucleosis(IM)children infected with Epstein-Barr virus(EBV)and their clinical significance.Methods A total of 100 children with EBV infected IM who were admitted to the Infectious Diseases Department of Children's Hospital of Soochow Uni-versity from September 2018 to August 2020 were selected as case group.A total of 60 healthy children who underwent physical examinationinthechildhealthcaredepartmentinthesamehospitalduringthesameperiodwereselectedascontrolgroup.The levels of lymphocyte subsets and cytokines(IL-6,IL-10,and γ-IFN)in peripheral blood of the children with IM and the con-trol group were tested by flow cytometry and enzyme-linked immunosorbent assay,respectively.Results The cytokine levels of IL-6,IL-10,and γ-IFN in the case group were higher than those in the control group(P<0.05 or 0.001).CD3+and the per-centage of CD3+CD8+in the case group were higher than those in the control group,while the percentages of CD3+CD4+,CD19+CD23+,CD3-CD(16+56)+,CD3-CD19+,and CD4+/CD8+ratio were lower than those in the control group,and the differences between the two groups were statistically significant(P<0.001).There were all statistically significant differences in the percentage of atypical lymphocytes,WBC,ALT,AST,and LDH between the two groups(P<0.05 or 0.01).There was no correlation between IL-6 and the percentage of atypical lymphocytes,WBC,plasma EBV-DNA viral load,ALT,and LDH in the case group(P>0.05),but there was a positive correlation with AST(P<0.05).There was no correlation between IL-10 and the percentage of atypical lymphocytes,WBC,plasma EBV-DNA viral load,ALT,AST,and LDH in the case group(P>0.05).In the case group,γ-IFN had no correlation with the percentage of atypical lymphocytes,WBC,ALT,LDH(P>0.05),but it was positively correlated with plasma EBV-DNA viral load and AST(P<0.05).In the case group,IL-6 was negatively correlated with CD3+,the percentage of CD3+CD8+(P<0.05 or 0.001),positively correlated with the percentages of CD3+CD4+,CD3-CD19+and CD4+/CD8+(P<0.05 or 0.001),but it was not correlated with the percentages of CD19+CD23+,and CD3-CD(16+56)+(P>0.05).In the case group,IL-10 was positively associated with the percentages of CD19+CD23+,CD3-CD19+percentage(P<0.05),negatively with CD3+(P<0.05),and it was not correlated with the percentages of CD3+CD4+,CD3+CD8+,CD3-CD(16+56)+,and CD4+/CD8+(P>0.05).In the case group,γ-IFN was negatively associ-ated with CD3+and the percentage of CD3+CD8+(P<0.05),but it had no correlation with the percentages of CD19+CD23+,CD3+CD4+,CD3-CD(16+56)+,CD3-CD19+,and CD4+/CD8+(P>0.05).Conclusion The immune changes and inflam-matory response of EBV-infected children with IM are related to the occurrence and development of the disease,IL-6,IL-10 and γ-IFN are involved in the immunological pathogenesis of IM.
