Effects of endoscopic tissue gel injection combined with ligation and octreotide on the coagulation function and rebleeding in the treatment of EGVB
Objective To explore the effects of endoscopic tissue gel injection and ligation combined with octreotide on coagulation function and rebleeding rate in the treatment of esophageal-gastric variceal bleeding(EGVB).Methods A total of 226 EGVB patients who met inclusion and exclusion criteria from January 2020 to January 2023 were selected as study sub-jects.They were divided into study group and control group according to random number table,with 113 patients in each group.The control group were treated with ligation combined with octreotide,while the study group were treated with endo-scopic tissue gel injection combined with ligation and octreotide.And then,therapeutic effects,blood transfusion volume,hemostatic time,hematemesis improvement time,coagulation functions(activated partial prothrombin time[APTT],pro-thrombin time[PT],coagulation time[TT]),portal vein hemodynamics(portal vein pressure[PVP],portal vein blood flow[PVF],portal vein blood flow velocity[PVV]),gastrointestinal hormone levels(gastrin-17[Gas-17],pepsin I[PG-Ⅰ],pepsinⅡ[PG-Ⅱ]),and rebleeding rates were compared between the two groups.Results The total effective rate of the study group(97.35%,110/113)was significantly higher than that of the control group(90.27%,102/113),and difference was statistically significant(P<0.05).Compared with the control group,the study group had less blood transfu-sion,hematemesis time,and hematemesis improvement time,and difference was statistically significant(P<0.001).After treatment,APTT,PT,and TT were shortened in both groups compared with those before treatment,and those in the study group were shorter than those in the control group,and difference was statistically significant(P<0.001).After treatment,PVP,PVF,and PVV in both groups decreased,and those in the study group decreased more significantly than those in the control group,and difference was statistically significant(P<0.001).After treatment,Gas-17,PG-Ⅰ,and PG-Ⅱ in the two groups decreased,and those in the study group also decreased more significantly than those in the control group,and difference was statistically significant(P<0.001).The rates of rebleeding in the study group were 0 and 5.31%(6/113)at 1 month and 3 months after treatment,respectively,which were lower than those in the control group(7.96%[9/113]and 13.27%[15/113]),and difference was statistically significant(P<0.05or0.01).Conclusion Endoscopictissuegelin-jection combined with ligation and octreotide is effective in the treatment of EGVB.It can improve coagulation function and bleeding symptoms,improve portal vein hemodynamics,reduce the rate of rebleeding,and may alleviate gastrointestinal function damage.
