6种筛查工具对中国老年人肌少症筛查准确性的Meta分析

目的 使用网状Meta分析比较6种常用的肌少症筛查工具对中国老年人肌少症的诊断价值,为临床实践选择合适的老年肌少症筛查工具提供参考.方法 全面检索中文数据库:中国知网(CNKI)、中国生物医学文献数据库(CBM)、万方数据库(Wanfang Date)、维普数据库(VIP),英文数据库Pubmed、Embase、Web of science,收集关于老年肌少症筛查工具的诊断性试验研究文献,检索时间截至2023年12月31日.将检索得到的文献剔除重复后,按照纳排标准进行初筛、复筛确定最终纳入的文献,自制数据提取表提取相关数据后,按照QUADAS-2工具进行文献质量评价.最后通过Stata 18.0软件进行Meta分析.结果 研究最终纳入了24篇文献,其中英文15篇,中文9篇,涉及6种筛查问卷,共计样本量17208例.网状Meta分析结果显示,在AWGS诊断标准下,6种肌少症筛查工具中,按照灵敏度累积排序概率曲线下面积(SUCRA)从高到低排序为MSRA-7、MSRA-5、CC、SARC-Calf、Ishii、SARC-F,特异度SUCRA值从高到低依次为SARC-F、SARC-Calf、MSRA-7、MSRA-5、CC、Ishii.结论 在常用的老年人肌少症筛查工具中,建议采用SARC-Calf问卷筛查工具进行初筛,但受研究数量和质量的限制,还需更多大样本、高质量的研究进一步证实.
Screening accuracy of six screening tools for sarcopenia in elderly Chinese patients:a network Meta-analysis
Objective To compare the diagnostic value of six commonly used sarcopenia screening tools for sarcopenia in elder-ly Chinese adults by network Meta-analysis,so as to provide a reference for selecting appropriate sarcopenia screening tools for eld-erly adults in clinical practice.Methods Chinese databases including China National Knowledge Infrastructure (CNKI),China Biology Medicine disc (CBMdisc),Wanfang Data,VIP Database as well as English databases including PubMed,EMbase,Web of science were comprehensively searched to collect diagnostic test research literature on screening tools for sarcopenia in the elder-ly,and the search was conducted as of December 31,2023.After removing duplicates from the retrieved literature,preliminary screening and rescreening were conducted according to inclusion and exclusion criteria to determine the final included literature.After extracting relevant data from a self-made data extraction table,the literature quality was evaluated using the QUADAS-2 tool.Then,Stata 18.0 software was used for Meta-analysis.Results The study finally included 24 articles,including 15 in English and 9 in Chinese,involving 6 screening questionnaires,with a total sample size of 17208 cases.The results of the network Meta-analy-sis showed that under the AWGS diagnostic criteria,among the six screening tools for sarcopenia,surface under the cumulative ranking curve (SUCRA) ranking from high to low were as follows:MSRA-7,MSRA-5,CC,SARC-Calf,Ishii and SARC-F.Spe-cific SUCRA values from high to low were as follows:SARC-F,SARC-Calf,MSRA-7,MSRA-5,CC and Ishii.Conclusion A-mong the commonly used screening tools for sarcopenia in the elderly,it is recommended to use the SARC-Calf questionnaire screening tool for preliminary screening,but due to the limitations of the number and quality of studies,more large samples and high-quality studies are needed to further confirm.
