摘要
目的 分析胸段食管癌放疗时双肺剂量学参数与计划参数的相关性.方法 选取2020年4月至2023年4 月医院收治的 99 例胸段食管癌放疗患者,采集放射性肺炎相关的剂量学参数如双肺V5、V10、V20、V30 和平均剂量(Dmean)等,选取计划参数如处方剂量(DP)、双肺体积(VBL)、靶区体积(VPTV)和靶区与肺重合体积(VOPL)等,分析胸段食管癌放疗中双肺剂量学参数与计划参数的相关性.结果 当双肺剂量学参数组合为第 1 典型变量(V1)、计划参数组合为第 1 典型变量(U1)时,两组典型变量的相关系数ρ=0.848,差异有统计学意义(P<0.05),其他组合的相关系数比较,差异无统计学意义(P>0.05).其中V1 包括双肺剂量学参数中的V5、V20、V30 和Dmean,U1 包括计划参数中的处方剂量(DP)、双肺体积(VBL)和靶区体积(VPTV).结论 V5、V20、V30 和Dmean均是重要的双肺剂量学参数,其中,物理师需密切关注Dmean.计划参数的影响因素包括DP、VBL和VPTV,其中VPTV是影响双肺剂量学参数的重要因素.
Abstract
Objective To analyze the correlation between bilateral lung dosimetric parameters and planning parameters during radiotherapy for thorax.Methods A total of 99 cases of thoracic esophageal cancer radiotherapy in hospital were selected from April 2020 to April 2023,and the dosimetric parameters related to radiation pneumonitis such as bilateral lung V5,V10,V20,V30,and mean dose(Dmean)were collected,and the planning parameters such as dose of prescription(DP),volume of bilateral lung(VBL),volume of planning target volume(VPTV),and volume of overlap between PTV and lung(VOPL)were selected,and analyzed the correlation between bilateral lung dosimetric parameters and planning parameters in radiotherapy for thoracic esophageal cancer.Results The correlation coefficient ρ=0.848 of the two canonical variables was statistically significant(P<0.05)when the combination of bilateral lung dosimetric parameters was the 1st canonical variable(V1)and the combination of planning parameters was the 1st canonical variable(U1),and the correlation coefficients of the other combinations were not statistically significant(P>0.05).Where V1 included V5,V20,V30,and Dmean in the bilateral lung dosimetric parameters,U1 included dose of prescription(DP),volume of bilateral lung(VBL),and volume of planning target volume(VPTV)in the planning parameters.Conclusions V5,V20,V30,and Dmean were important bilateral lung dosimetric parameters in the thoracic esophageal cancer radiotherapy,where Dmean needs to be closely examined by physicists.The influencing factors in the planning parameters included DP,VBL,and VPTV,with VPTV being the most prominent factor,among them,the VPTV is an important factor influencing the dosimetric parameters in bilateral lung.
