目的 探讨主动脉内球囊反搏泵(IABP)在急性心肌梗死(AMI)伴心源性休克(CS)患者经皮冠状动脉介入术(PCI)不同时机的应用.方法 选取 2021 年 1 月至2023 年 8 月于医院拟行PCI治疗的 70 例AMI伴CS患者,随机分为两组,各 35 例.两组均给予吸氧、心电监护、基础药物及急诊PCI治疗.试验组和对照组分别于PCI术前和术后置入IABP.比较两组术前和术后 24 h的血流动力学指标[平均动脉压(MAP)、心率(HR)和尿量]水平,术前和术后 7 d的心肌损伤标志物[血清肌酸激酶同工酶(CK-MB)和肌钙蛋白I(cTnI)]水平,术前和术后 3 个月的心功能指标[左心室射血分数(LVEF)、肺毛细血管契压(PCWP)和心脏指数(CI)]水平,以及术后 30 d内的病死率.结果 术后 24 h,两组MAP和尿量均高于术前,HR低于术前,且试验组MAP和尿量均高于对照组,HR低于对照组,差异均有统计学意义(P<0.05);术后7 d,两组血清CK-MB和cTnI水平均低于术前,且试验组低于对照组,差异均有统计学意义(P<0.05);术后 3 个月,两组LVEF和CI水平均高于术前,PCWP水平均低于术前,且试验组LVEF和CI水平高于对照组,PCWP水平低于对照组,差异均有统计学意义(P<0.05).术后 30 d内,两组病死率比较,差异无统计学意义(P>0.05).结论 AMI伴CS患者行PCI术前应用IABP较术后应用IABP更能改善血流动力学指标,减轻心肌受损,提高心功能.
Application of Intra Aortic Balloon Pump in Different Timing of Percutaneous Coronary Intervention in Patients with Acute Myocardial Infarction and Cardiogenic Shock
Objective To explore the application of intra aortic balloon pump(IABP)in different timing of percutaneous coronary intervention(PCI)in patients with acute myocardial infarction(AMI)and cardiogenic shock(CS).Methods With the selection of seventy AMI patients with CS who planned to undergo PCI treatment in the hospital from January 2021 to August 2023,they were randomly divided into two groups,with 35 patients in each group.Both groups received oxygen therapy,electrocardiogram monitoring,basic medication,and emergency PCI treatment.IABP was implanted before and after PCI in the experimental group and control group,respectively.The levels of hemodynamic indicators[mean arterial pressure(MAP),heart rate(HR)and urine volume]before and 24 h after surgery,myocardial injury markers[creatine kinase MB(CK-MB)and cardiac troponin I(cTnI)]before and 7 days after surgery,cardiac function indicators[left ventricular ejection fraction(LVEF),pulmonary capillary wedge pressure(PCWP),and cardiac index(CI)]before and 3 months after surgery in both groups were compared,as well as mortality within 30 days after surgery.Results 24 h after surgery,MAP and urine volume of both groups were higher than before surgery,HR was lower than before surgery,and MAP and urine volume of experimental group were higher than control group.In addition,HR was lower than control group,the differences were statistically significant(P<0.05);7 days after surgery,serum CK-MB and cTnI levels in 2 groups were lower than before surgery,and the experimental group was lower than the control group,the differences were statistically significant(P<0.05);Three months after surgery,LVEF and CI levels in both groups were higher than before surgery,PCWP levels were lower than before surgery,LVEF and CI levels in experimental groups were higher than control group,and PCWP levels were lower than control group,with statistical significance(P<0.05).Within 30 days after operation,there was no significant difference in mortality between the two groups(P>0.05).Conclusion The application of IABP before PCI in AMI patients with CS can improve hemodynamic indexes,reduce myocardial damage and improve cardiac function more than that after PCI.
