Correlation between Physical Measurements and Cardiac Structure and Function in Obese Children
Objective To explore the relationship between changes in cardiac structure and function and the degree of obesity in obese children. Methods Seventy-six obese children admitted to the Department of Child Endocrinology of Jiujiang Maternal and Child Health Hospital from January to December 2023 were selected,of which 23 were in the mildly obese group,36 were in the moderately obese group,17 were in the severely obese group, and 50 healthy children who underwent physical examination in the health care department during the same period were selected as the control group. The correlation between basic information, physical measurements, cardiac ultrasound parameters and physical measurements and cardiac ultrasound parameters of the children in the 4 groups were compared,as well as the predictive value of different measurements for cardiac structure and function. Results The onset time, blood pressure, electrocardiogram, fasting insulin, total cholesterol(TC),triacylglycerol(TG),uric acid(UA),and low-density lipoprotein(HDL-c)were higher than those of the control group in the three groups of obese children(P<0.05),and HDL-c was lower than that of the control group(P<0.05);the comparison of the heights, upper volumes, lower volumes, and upper and lower volumes among the four groups of children did not have statistically significant differences(P>0.05);BMI,body mass, chest circumference,AC,WC,hip circumference, hip ratio(WHR)and waist to height ratio(WHtR)of children in the obese group were higher than those in the control group(P<0.05),and those in the moderately obese group and the severely obese group were higher than those in the mildly obese group(P<0.05),and those in the severely obese group were higher than those in the moderately obese group(P<0.05);the end-systolic volume(ESV),left ventricular end-diastolic volume(EDV),left ventricular posterior wall diastolic thickness(LVPWd),left ventricular posterior wall systolic thickness(LVPWs),interventricular septal diastolic thickness(IVSd),left ventricular mass index(LVM),and stroke volume(SV)were higher in the 3 groups than in the control group(P<0.05);in the 3 groups of obese children, the BMI,body mass, chest circumference, waist circumference, abdominal circumference, hip circumference, waist to WHR,WHtR were higher than those of the control group(P<0.05);BMI was positively correlated with SV(r=0.564,P<0.001).Conclusion The higher the degree of obesity in children, the more significant the changes in cardiac structure and function, and the higher the risk of heart disease.
