目的 探讨网格定位器在股骨近端防旋髓内钉内固定术治疗股骨粗隆间骨折患者中的应用价值.方法 选取2022年1月至2023年12月医院收治的60例股骨粗隆间骨折患者作为研究对象,按照随机数字表法分为对照组与试验组,各30例.对照组采用股骨近端防旋髓内钉内固定术治疗,试验组在对照组基础上术中应用网格定位器.比较两组的手术指标(手术时间、术中出血量、切口长度、恢复负重时间、住院时间、骨愈合时间)、术前和术后3个月的髋关节功能(采用Harris髋关节评分量表评估)及术后并发症发生率.结果 试验组手术时间、恢复负重时间、住院时间、骨愈合时间均短于对照组,术中出血量低于对照组,切口长度短于对照组,差异均有统计学意义(P<0.05).术前,两组Harris髋关节评分量表评分比较,差异无统计学意义(P>0.05);术后3个月,试验组Harris髋关节评分量表评分高于对照组,差异有统计学意义(P<0.05).两组术后并发症发生率比较,差异无统计学意义(P>0.05).结论 股骨粗隆间骨折患者行股骨近端防旋髓内钉内固定术中采用网格定位器,可缩短手术时间、降低术中出血量、促进患者术后恢复、提高患者髋关节功能,且不增加并发症风险.
The Application Effect of Grid Locator in the Treatment of Intertrochanteric Fractures in Patients with Proximal Femoral Nail Antirotation Internal Fixation Surgery
Objective To explore the application value of grid locator in proximal femoral nail antirotation internal fixation for patients with femoral intertrochanteric fracture.Methods With the selection of 60 patients with femoral intertrochanteric fracture admitted to the hospital from January 2022 to December 2023 as the research subjects,they were divided into the control group and the experimental group using a random number table method,with 30 cases in each group.With the use of proximal femoral nail antirotation internal fixation for treatment in the control group,and the application of grid locators during surgery on the basis of the control group in the experimental group,the surgical indicators(operation time,intraoperative blood loss,incision length,recovery time for weight-bearing,hospital stay time and bone healing time),hip joint function(assessed by Harris hip score scale)before and 3 months after operation,and the incidence of postoperative complications were compared between the two groups.Results The operation time,recovery time for weight-bearing,hospital stay time and bone healing time of experimental group were shorter than those of control group,the intraoperative blood loss was lower than that of control group,and the incision length was shorter,with statistical significance(P<0.05).Before surgery,there was no statistically significant difference in Harris scores between the two groups(P>0.05);Three months after surgery,the Harris score of the experimental group was higher than that of the control group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in the incidence of postoperative complications between the two groups(P>0.05).Conclusion The use of grid locators during proximal femoral nail antirotation internal fixation surgery for patients with femoral intertrochanteric fractures can shorten surgical time,reduce intraoperative bleeding,promote postoperative recovery,improve hip joint function,and not increase the risk of complications.
