益生菌、维生素D辅助孟鲁司特钠治疗肥胖型哮喘患儿的效果及其对Th17/Treg、T-bet/GATA-3表达、瘦素的影响

目的 探讨益生菌、维生素D辅助孟鲁司特钠治疗肥胖型哮喘患儿的效果及机制.方法 选取2020年9 月—2023 年4 月中国人民解放军联勤保障部队第九八〇医院儿科收治肥胖型哮喘患儿90 例,按随机数字表法分为3 组,每组30 例,均采取孟鲁司特钠常规治疗,于此基础上对照1 组加用益生菌,对照2 组加用维生素D,观察组加用益生菌、维生素D.观察患儿疗效、安全性及治疗前后肺功能[第 1 秒用力呼气容积占预测值百分比(FEV1%pred)、呼出气一氧化氮(FeNO)、呼气流速峰值占预测值百分比(PEF%pred)]、Th17/Treg细胞因子[白介素-10(IL-10)、白介素-6(IL-6)、白介素-17(IL-17)]、T细胞中T-box(T-bet)/GATA结合蛋白 3(GATA-3)表达、Th1/Th2 细胞因子[白介素-4(IL-4)、γ干扰素(IFN-γ)、IFN-γ/IL-4]、脂肪细胞因子[瘦素、脂联素、丝氨酸蛋白酶抑制剂(vaspin)]变化.结果 观察组总有效率(100.00%)与对照1 组(90.00%)、对照2 组(86.67%)比较,差异均无统计学意义(P>0.05);治疗后观察组PEF%pred、FEV1%pred显著高于对照 1 组、对照 2 组,FeNO低于对照 1 组、对照 2 组(F/P = 19.134/<0.001、9.613/<0.001、14.069/<0.001),血清IL-6、IL-17 水平低于对照 1 组、对照 2 组(F/P =27.374/<0.001、45.591/<0.001),IL-10 水平高于对照1 组、对照2 组(F/P =58.689/<0.001);T-bet、T-bet/GATA-3 高于对照1 组、对照2 组,GATA-3 低于对照 1 组、对照 2 组(F/P =14.137/<0.001、92.423/<0.001、13.984/<0.001);血清IFN-γ、IFN-γ/IL-4 水平高于对照 1 组、对照 2 组(F/P =103.381/<0.001、749.586/<0.001),IL-4 水平低于对照 1组、对照2 组(F/P =114.110/<0.001),而对照1 组、对照 2 组上述指标比较差异均无统计学意义(P均>0.05);治疗后血清瘦素、vaspin水平观察组<对照1 组<对照2 组(F/P =69.470/<0.001、17.835/<0.001),脂联素水平观察组>对照1 组>对照2 组(F/P =47.148/<0.001).结论 益生菌、维生素D辅助孟鲁司特钠治疗可改善肥胖型哮喘患儿肺功能,对脂肪细胞因子、T-bet/GATA-3 表达、Th1/Th2 及Th17/Treg细胞产生调控作用,疗效确切.
Objective To investigate the effects and mechanisms of probiotics and vitamin D assisted montelukast sodium in the treatment of obese children with asthma.Methods A total of 90 obese asthmatic children were selected from the Pediatric Department of the 980th Hospital of the Chinese People's Liberation Army Joint Logistics Support Force from September 2020 to April2023,and were randomly divided into three groups of 30 cases each.All groups received conven-tional treatment with montelukast sodium,and on this basis,group 1 was given probiotics,group 2 was given vitamin D,and the observation group was given probiotics and vitamin D.Observe the efficacy and safety of the children,as well as the changes in lung function before and after treatment[percentage of forced expiratory volume in one second(FEV1%)pred,exhaled nitric oxide(FeNO),percentage of peakexpiratory flow(PEF%)pred],Th17/Treg cytokines[interleukin-10(IL-10),interleukin-6(IL-6),interleukin-17(IL-17)],T-box(T-bet)/GATA binding protein 3(GATA-3)expression of T cells,Th1/Th2 cytokines[interleukin-4(IL-4),interferon-gamma(IFN-γ),IFN-γ/IL-4],adipocytokines[leptin,adiponectin,serine pro-tease inhibitor(vaspin)].Results The total effective rate of the observation group was 100.00%,which was not signifi-cantly different from that of the control group 1 and control group 2(90.00%and 86.67%,respectively)(P>0.05);after treatment,the PEF%pred and FEV1%pred of the observation group were higher than those of the control group 1 and con-trol group 2(F/P=19.134/<0.001 and 9.613/<0.001),and FeNO,the serum levels of IL-6 and IL-17 were lower than those of the control group 1 and control group 2(F/P=14.069/<0.001,27.374/<0.001,45.591/<0.001),while the serum level of IL-10 was higher than that of the control group 1 and control group 2(F/P=58.689/<0.001);the T-bet and T-bet/GATA-3 levels were higher than those of the control group 1 and control group 2,while the GATA-3 level was lower than that of the con-trol group 1 and control group 2(F/P=14.137/<0.001,92.423/<0.001,13.984/<0.001);the serum levels of IFN-γ and IFN-γ/IL-4 were higher than those of the control group 1 and control group 2(F/P=103.381/<0.001 and 749.586/<0.001),while the serum level of IL-4 was lower than that of the control group 1 and control group 2(F/P=114.110/<0.001);there was no significant difference in the above indicators between the control group 1 and control group 2(all P>0.05);after treatment,the serum levels of leptin and vaspin in the observation group were lower than those of the control group 1 and control group 2(F/P=69.470/<0.001 and 17.835/<0.001),while the adiponectin level in the observation group was higher than that of the control group 1 and control group 2(F/P=47.148/<0.001).Conclusion Probiotics and vitamin D assisted monte-lukast sodium treatment can improve lung function in obese children with asthma,regulate adipocyte cytokines,T-bet/GA-TA-3 expression,Th1/Th2,and Th17/Treg cells,and have a definite therapeutic effect.
