Relationship between serum miR-183-3p and miR-495-3p expression with clinical pathological characteristics and prognosis of esophageal cancer patients

目的 探讨血清miR-183-3p、miR-495-3p表达与食管癌患者临床病理特征及预后的关系.方法 选择2017年12月-2020年12月解放军总医院第三医学中心胸外科诊治食管癌患者105例作为食管癌组,随访3年根据患者生存情况分为存活亚组80例和死亡亚组25例;另选择医院同期健康体检者105例作为健康对照组.采取qRT-PCR 法检测血清 miR-183-3p、miR-495-3p 表达;绘制 Kaplan-Meier 曲线分析血清 miR-183-3p、miR-495-3p 与食管癌患者预后的关系;ROC曲线分析血清miR-183-3p、miR-495-3p对食管癌患者预后的预测价值;Cox风险回归模型分析影响食管癌患者预后的因素.结果 食管癌组血清miR-183-3p表达高于健康对照组,血清miR-495-3p表达低于健康对照组(t/P=8.760/<0.001、4.054/<0.001);死亡亚组血清miR-183-3p表达高于存活亚组,血清miR-495-3p表达低于存活亚组(t/P=3.655/<0.001、4.210/<0.001);miR-183-3p高表达患者低分化程度、AJCC临床Ⅲ~Ⅳ期比例高于miR-183-3p低表达患者,差异均有统计学意义(x2/P=10.680/0.001、4.246/0.039);miR-495-3p高表达患者中高分化程度、AJCC临床Ⅰ~Ⅱ期比例高于miR-495-3p低表达患者,差异均有统计学意义(x2/P=5.341/0.021、9.320/0.002).miR-183-3p 高表达患者 3 年生存率(67.92%)低于 miR-183-3p 低表达患者(84.62%),miR-495-3p 低表达患者 3 年生存率(64.81%)低于 miR-495-3p 高表达患者(88.24%)(x2/P=5.279/0.022、9.351/0.002).血清miR-183-3p、miR-495-3p及二者联合预测食管癌患者预后的AUC分别为0.763、0.737、0.864,二者联合优于各自单独预测效能(Z/P=2.564/0.010、2.011/0.043).低分化、AJCC 临床分期 Ⅲ~Ⅳ期、miR-183-3p 高表达、miR-495-3p 低表达为影响食管癌患者预后的危险因素[HR(95%CI)=6.628(2.278~19.288)、5.803(2.261~14.897)、5.130(2.187~12.034)、6.152(1.951~19.401)].结论 食管癌患者血清 miR-183-3p 呈高表达,miR-495-3p 呈低表达,二者表达与患者肿瘤分化程度和AJCC临床分期有关,且可作为预测预后的生物指标.
Objective To explore the relationship between serum miR-183-3 p and miR-495-3p expression and clini-cal pathological characteristics and prognosis of esophageal cancer patients.Methods A total of 105 patients with esopha-geal cancer admitted to The Third Medical Center of the General Hospital of the People's Liberation Army from December 2017 to December 2020 as the esophageal cancer group,and another 105 healthy individuals who underwent physical exami-nations as the control group.The qRT-PCR method was applied to detect the expression of serum miR-183-3p and miR-495-3p.Kaplan-Meier curve was plotted to analyze the relationship between miR-183-3p,miR-495-3p and the prognosis of esophageal patients.ROC was applied to analyze the predictive value of miR-183-3p and miR-495-3p for the prognosis of esophageal cancer patients.COX risk regression model was applied to analyze the factors affecting the prognosis of esoph-ageal cancer patients.Results The expression level of miR-183-3p in the esophageal cancer group was higher than that in the control group,while the expression level of miR-495-3p was lower than that in the control group(t=8.760,4.054,P<0.001).The expression of serum miR-183-3p in the death group was higher than that in the survival group,while the expres-sion of serum miR-495-3p was lower than that in the survival group(t=3.655,4210,P<0.001).Patients with high expression of miR-183-3p had an increased degree of low differentiation and the proportion of clinical stage Ⅲ-Ⅳ of AJCC,while pa-tients with low expression of miR-183-3p had an increased degree of middle and high differentiation and the proportion of clinical stage Ⅰ-Ⅱ of AJCC,and the differences were statistically significant(x2=10.680,P=0.001;x2=4.246,P=0.001).Patients with high expression of miR-495-3p had an increased degree of middle and high differentiation and the proportion of clinical stage Ⅰ-Ⅱ of AJCC,while patients with low expression of miR-495-3p had an increased degree of low differentia-tion and the proportion of clinical stage Ⅲ-Ⅳ of AJCC,and the differences were statistically significant(x2=5.341,P=0.021;x2=9320,P=0.002).The 3-year survival rate of patients with high expression of miR-183-3p(67.92%)was lower than that of patients with low expression of miR-183-3p(84.62%)(x2=5.279,P=0.022),and the 3-year survival rate of patients with low expression of miR-495-3p(64.81%)was lower than that of patients with high expression of miR-495-3p(8824%)(x2=9351,P=0.002).The AUC of serum miR-183-3p,miR-495-3p and mir-495-3p in predicting prognosis of patients with esophageal cancer were 0.763,0.737 and 0.864,respectively,and the AUC of combined was superior to that of their individu-al predictive efficacy(Z=2.564,2.011,P=0.010,0.043).Low differentiation,AJCC clinical stage Ⅲ-Ⅳ,high expression of miR-183-3p,and low expression of miR-495-3p were risk factors affecting the prognosis of esophageal cancer patients[HR(95%CI)=6.628(2278-19288),5.803(2261-14.897),5.130(2.187-12.034),6.152(1.951-19.401)].Conclusion Serum miR-183-3p is highly expressed and miR-495-3p is low in esophageal cancer patients.The expression of miR-183-3p and miR-495-3p is related to the degree of tumor differentiation and clinical stage of AJCC in patients,and can be used as biological indi-cators to predict prognosis.
