目的 了解本地区布鲁氏菌感染患者的临床分布及流行病学特征,为布鲁氏菌感染的早期诊断和临床治疗提供参考.方法 以玉溪市人民医院45例布鲁菌感染患者为研究对象,回顾性分析患者的基本信息、临床资料和实验室检查结果.结果 本院布鲁菌感染患者大多中年,男性居多;患者多为农民或自由职业者,部分存在牛羊接触史;患者临床症状以发热及腰、下肢关节疼痛较为主,本院多个科室均有患者;患者血常规白细胞指标多为正常、嗜酸性粒细胞可出现降低、炎性指标、肝功能指标出现不同程度的升高,本院主要采用"利福平+多西环素"联合治疗.结论 提示玉溪地区有布鲁氏菌感染病例,存在感染的风险,避免与牛羊接触,高度关注发热及腰、下肢关节疼痛患者的实验室指标,防止误诊漏诊,一旦确诊及时治疗.
Clinical distribution and epidemiological analysis of 45 cases of brucella infection in Yuxi area
Objective To understand the clinical distribution and epidemiological characteristics of patients with brucella infection in this area,so as to provide reference for early diagnosis and clinical treatment of brucellosis.Methods 45 patients with Brucella infection in Yuxi People's Hospital were selected as the research object,and their basic information,clinical data and laboratory examination results were retrospectively analyzed.Results Most of the patients with Brucella infection were middle-aged and male in our hospital.Most of them were farmers or freelance workers,and some of them had a history of exposure to cattle and sheep.The main clinical symptoms of the patients were fever and waist and lower limb joint pain,and there were patients in multiple departments of our hospital.The white blood cell index of the patients was normal,the eosinophils might be decreased,and inflammatory index and liver function index were increased in different degrees.Our hospital mainly used"rifampicin+doxycycline"combined treatment.Conclusions It is suggested that there are brucella infection cases in Yuxi area,there is a risk of infection,avoid contact with cattle and sheep,pay more attention to laboratory indicators of patients with fever and waist and lower limb joint pain,and prevent misdiagnosis and missed diagnosis.Once diagnosed,timely treatment is needed.
