摘要
王彦彬按痈论治儿童鼻源性反复咳嗽,辨证上抓住黏性鼻涕、伴平卧或体位变化咳嗽加重、间断咳嗽几声主症,就能定固定专方专药甚至专量,疗效显著,定病机为肺胃胆郁热,浊涕堵清窍,治疗上按痈论治,以清热消痈和排脓外出,清热消痈从肺、胆、胃热和清热解毒入手,分别选用苇茎汤、霍胆丸及消食清热药,和重用蒲公英,排脓选用白芷、天花粉,重用黄芪托脓外出.虽是热证,用药上并非一味苦寒药,而是寒热并用,避免寒伤脾胃,主张配合外治法优势互补,强调饮食调护是关键.
Abstract
Professor Wang Yanbin treats children with recurrent rhinogenic cough with carbuncle theory,determining special medicine and prescription by grasping the symptoms such as sticky nasal mucus,exacerbated cough when lying down or changing posture,and intermittent coughing.The pathogenesis is identified as heat stagnation in the lung,stomach,and gallbladder,leading to obstruction of the clear orifices.The treatment involves clearing heat,resolving carbuncles and expelling pus.Clearing heat and resolving carbuncles is approached by targeting the lung,gallbladder,and stomach,using reed stem decoction,Huodan pills and medicines which can promote digestion and harmonize stomach,respectively,with dandelion used in larger than usual amounts.For expelling pus,Angelica Dahurica and radices trichosanthis are used to reduce swelling and drain pus,and milkvetch root is used in larger than usual amounts to aid pus expulsion.The whole prescription attaches importance to the balance of cold and heat and avoids cold injuring the spleen and stomach.There is also special medicine and prescription for concurrent pneumonia and preventing recurrence,advocating for simple,effective,and affordable external treatments,and emphasizing nursing methods.The article concludes with four effective case studies,detailing how to identify the main symptoms and distinguish rhinogenic cough,as well as how to adjust medications according to different symptoms for verification.
