目的 观察帕米膦酸二钠对肺癌骨转移性疼痛的及骨纤维结构的影响。方法 南阳医学高等专科学校附属中医院 2021 年 1 月至 2022 年 12 月收治的 109 例肺癌骨转移患者,所有患者均伴有不同程度的骨痛症状。采用随机数字表法对入组患者进行分组,即常规组(54 例)和试验组(55 例)。常规组予以内科综合止痛治疗,试验组在常规组的治疗基础上采用帕米膦酸二钠配合治疗,比较两组患者的骨纤维结构改善情况、疼痛介质变化情况、骨痛缓解情况及预后情况。结果 治疗前,两组患者的骨纤维结构相关指标差异无统计学意义(P>0。05);治疗后,试验组的骨钙素、Ⅰ型胶原C端肽、Ⅰ型胶原氨基端前肽均低于常规组(P<0。05)。治疗前,两组患者的疼痛介质差异无统计学意义(P>0。05);治疗后,试验组的前列腺素、血清乳酸均低于常规组(P<0。05)。治疗前,两组患者的骨痛程度差异无统计学意义(P>0。05);在不同治疗方案下,试验组治疗 1~3 个周期后的视觉模拟疼痛量表评分均低于常规组(P<0。05)。在不同治疗方案下,两组不良反应发生率比较,差异无统计学意义(P>0。05);试验组的肺癌患者生存质量测定量表中,身体状况评分,社交/家庭状况评分,情感状况评分及功能状况评分均高于常规组(P<0。05)。结论 帕米膦酸二钠能有效改善肺癌骨转移患者的骨纤维结构,可通过下调疼痛介质水平而缓解骨痛症状,用药安全性良好,对改善患者远期生活质量有积极意义。
Study on the Effect of Pamidronate Disodium on Bone Metastasis Pain and Bone Fiber Structure in Lung Cancer Patients
Objective To observe the effect of pamidronate disodium on bone metastasis pain and bone fiber structure in lung cancer patients.Methods The study included 109 patients with lung cancer bone metastasis admitted to The Affiliated Hospital of Traditional Chinese Medicine of Nanyang Medical College from January 2021 to December 2022,all patients were accompanied with bone pain symptoms of different degrees.The patients were divided into the conventional group(54 cases)and the experimental group(55 cases)by random number table method.The conventional group was given comprehensive medical pain relief treatment,the experimental group was treated with pamidronate disodium in combination with the conventional group,and the improvement of bone fiber structure,changes in pain mediators,bone pain relief,and prognosis of the two groups of patients were compared.Results Before treatment,there was no statistical difference in bone fiber structure between the two groups(P>0.05);after treatment,the osteocalcin,C-terminal peptide of type Ⅰ collagen,procollagen type Ⅰ N-terminal propeptide of the experimental group were lower than the conventional group,with statistically significant differences(P<0.05).Before treatment,there was no statistical difference in pain media between the two groups(P>0.05);after treatment,the prostaglandin,serum lactate of the experimental group were lower the conventional group,and the difference was significant(P<0.05).Before treatment,There was no statistical difference in the degree of bone pain between the two groups(P>0.05);under different treatment regimens,the visual analogue scale after 1-3 cycle of treatment of the experimental group were lower than conventional group,and the difference was significant(P<0.05).Under different treatment regimens,there was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05);in the Functional Assessment of Cancer Therapy-lung cancer,the physical well-being,social/family well-being,emotional well-being,functional well-being of the experimental group were all higher than conventional group and the difference was significant(P<0.05).Conclusion Pamidronate disodium can effectively improve the bone fiber structure of lung cancer patients with bone metastasis,and alleviate bone pain symptoms by down-regulating the level of pain mediators.This drug has high safety and is also of positive significance for improving the long-term quality of life of patients.
