Analysis of Influencing Factors of Hypothyroidism in Patients with Gestational Diabetes and Its Influence on Pregnancy Outcome
Objective To study the influencing factors of hypothyroidism in patients with gestational diabetes and its influence on pregnancy outcome.Methods From March 2022 to March 2023,80 pregnant diabetes patients with hypothyroidism who were admitted to Jian'an District Maternal and Child Health Care Hospital in Xuchang City were selected and included in the hypothyroidism group,and 80 pregnant diabetes patients without hypothyroidism during the same period were included in the non hypothyroidism group.The data of the two groups(including age,pre pregnancy body mass index,education level,marital status,pregnancy times,birth times,abortion,smoking or exposure to smoking)were compared,To analyze the influencing factors of hypothyroidism in pregnant women with diabetes.Results The incidence of obesity,history of miscarriage,smoking,or exposure to smoking environment in the hypothyroidism group were higher than that in the non hypothyroidism group,and the prothrombin time(PT)and thrombin time(TT)were higher than those in the non hypothyroidism group,with statistical significance(P<0.05);there were no statistically significant difference(P>0.05)in age,pregnancy,parity,education level,marital status,fasting blood glucose,2-hour postprandial blood glucose,activated partial thromboplastin time,antithrombin Ⅲ,and fibrinogen between the two groups;the results of logistic regression analysis showed that obesity(OR=4.529,95%confidence interval=1.226-16.728),history of miscarriage(OR=4.385,95%confidence interval=1.386-13.869),smoking or exposure to smoking environment(OR=6.217,95%CI=1.331-29.032),PT(OR=1.468,95%CI=1.117-1.930)and TT(OR=1.709,95%CI=1.281-2.280)were the influencing factors for hypothyroidism in patients with gestational diabetes(P<0.05).The incidence of premature rupture of membranes,premature birth,and fetal distress in the hypothyroidism group was higher than that in the non hypothyroidism group,and the difference was statistically significant(P<0.05).Conclusion Obesity,abortion history,smoking or exposure to smoking environment,prolonged PT and TT are the influencing factors of hypothyroidism in patients with gestational diabetes,which have a negative impact on pregnancy outcome.
