鼻窦排脓饮在脾胃湿热型慢性鼻窦炎行功能性鼻内镜手术患者中的应用效果

摘要
目的 探讨鼻窦排脓饮在脾胃湿热型慢性鼻窦炎(CRS)行功能性鼻内镜手术(FESS)患者中的应用效果.方法 选取 2020 年 6 月至 2022 年 12 月于新余市中医院就诊的CRS患者 96 例,采用随机数字表法分为两组,各 48 例.两组均行FESS,对照组采用注射用阿莫西林钠克拉维酸钾联合糠酸莫米松鼻喷剂治疗,观察组加用鼻窦排脓饮治疗,两组均连续治疗 4 周.对比两组临床疗效、鼻黏膜形态、鼻黏膜纤毛功能、炎症因子水平、中医证候积分及不良反应.结果 观察组临床总有效率较对照组高,治疗后Lund-Kennedy评分、中医证候积分及超敏C反应蛋白(hs-CRP)、白介素-6(IL-6)水平较对照组低,清除速度、清除率较对照组高(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05).结论 行FESS的脾胃湿热型CRS患者应用鼻窦排脓饮治疗效果较佳,可促进鼻黏膜形态及功能恢复,减轻炎症反应,促进疾病恢复.
Abstract
Objective To investigate the effect of sinus drainage fluid in patients with chronic sinusitis(CRS)with damp-heat type of spleen and stomach undergoing functional endoscopic nasal surgery(FESS).Methods A total of 96 patients with CRS in Xinyu Traditional Chinese Medicine Hospital from June 2020 to December 2022 were selected and divided into two groups with 48 patients in each group by random number table method.Both groups were treated with FESS,the control group was treated with amoxicillin sodium clavulanate potassium combined with mometasone furoate nasal spray,the observation group was treated with bidou painong decoction based on the control group,both groups were treated continuously for 4 weeks.The clinical efficacy,nasal mucosa morphology,nasal mucociliary function,inflammatory factor level,TCM syndrome score and adverse reactions were compared between the two groups.Results The total effective rate of observation group was higher than that of control group(P<0.05).After treatment,the Lund Kennedy score,and the levels of hypersensitive C-reactive protein,interleukin-6 and TCM syndrome score in observation group were lower than those in control group(P<0.05),the clearance speed and clearance rate in observation group were higher than those in control group.There was no difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion In patients with CRS with damp-heat type of spleen and stomach,the effect of sinus drainage fluid is better,which can promote the recovery of nasal mucosa morphology and function,reduce inflammation and promote the recovery of disease.
