Establishment and Effect Evaluation of Whole-process Pharmaceutical Care Model for Elderly Patients with Coronary Heart Disease
Objective To estabilsh a whole-process pharmaceutical care model provided by clinical pharmacists for elderly patients with coronary heart disease and explore its impact on the satety and effectiveness of medication after discharge.Methods 110 cases of elderly patients over 65 years old who were admitted to the cardiology department of Shangrao People's Hospital from January 2022 to January 2023 and were diagnosed with coronary heart disease by coronary angiography were randomly divided into control group and experimental group,with 55 cases in each group.The control group was given routine pharmaceutical care during hospitalization;the experimental group,pharmacists provided whole-process pharmaceutical care during hospitalization,and after discharge,pharmacists continued to give pharmaceutical intervention for patients,including:optimizing drug treatment plan,establishing pharmaceutical care files,making follow-up plans,tracking and follow-up recording patients'post-discharge medication compliance,incidence of adverse reactions,self-health management education,psychological counseling,etc.Results Six months after discharge,the treatment objective,awareness rate of drug related knowledge,emotion management and clinical pharmacist service satisfaction of experimental group were significantly higher than those of control group(P<0.05).One year after discharge,the experimental group was significantly better than the control group in terms of medication compliance,incidence of drug treaty-related problems,understanding of the condition,treatment goal,awareness rate of drug related knowledge,emotional management and clinical pharmacist service satisfaction,with statistical significance(P<0.05),and the control of risk factors in the experimental group was significantly improved.The experimental rate of blood glucose,blood pressure and blood lipids were significantly higher than that of control group,and the differences were significant(P<0.05).Conclusion The implementation of whole-process pharmaceutical care by clinical pharmacists for elderly patients with coronary heart disease can effectively improve the medication compliance of elderly patients,thereby improving the effect of drug therapy,reducing risk factors,improving the level and quality of pharmaceutical care,ensuring patients'rational drug use,and improving patients'satisfaction with medical treatment.
