目的 分析甲泼尼龙琥珀酸钠联合噻托溴铵对慢性阻塞性肺疾病急性加重期(AECOPD)患者血气指标、炎症指标的改善作用。方法 回顾性选取商丘市中医院2020年10月至2023年10月AECOPD患者120例,根据治疗方法不同分为试验组、常规组,各60例。常规治疗基础上,常规组予以噻托溴铵,试验组接受甲泼尼龙琥珀酸钠联合噻托溴铵治疗,均治疗2周。比较两组临床疗效、血气指标、肺功能、炎症指标及不良反应。结果 试验组总有效率88。33%较常规组73。33%高(P<0。05);与治疗前相比,两组治疗2周后血动脉血氧分压(PaO2)、血pH值升高,血二氧化碳分压(PaCO2)下降,且试验组改善幅度高于常规组(P<0。05);治疗2周后,两组用力1s呼气容积(FEV1)、FEV1/用力肺活量(FEV1/FVC)、最大呼气流速(PEF)均呈升高趋势,但试验组升高幅度多于常规组(P<0。05);治疗2周后,两组血清白细胞介素-8(IL-8)、肿瘤坏死因子-α(TNF-α)、补体1q/肿瘤坏死因子相关蛋白5(CTRP5)、趋化因子配体12(CXCL12)水平均呈下降趋势,但试验组下降幅度多于常规组(P<0。05);两组不良反应对比,差异无统计学意义(P>0。05)。结论 甲泼尼龙琥珀酸钠联合噻托溴铵能有效改善AECOPD患者血气指标、肺功能,减轻肺部炎症损伤,疗效确切,安全可靠。
Methylprednisolone Sodium Succinate Combined with Tiotropium Bromide on the Improvement of Blood Gas and Inflammation Indicators in Patients with AECOPD
Objective To analyze the effect of methylprednisolone sodium succinate combined with tiotropium bromide on blood gas and inflammation indicators in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods A total of 120 patients with AECOPD in Shangqiu Hospital of Traditional Chinese Medicine from October 2020 to October 2023 were retrospectively selected,and they were divided into experimental group and conventional group according to different treatment methods,with 60 cases in each group.On the basis of conventional treatment,the conventional group was treated with tiotropium bromide,and the experimental group was treated with methylprednisolone sodium succinate combined with tiotropium bromide,both of which were treated for 2 weeks.The clinical efficacy,blood gas indexes,lung function,inflammatory indexes and adverse reactions were compared between the two groups.Results The total effective rate of the experimental group was 88.33%higher than that of the conventional group(73.33%)(P<0.05);compared with the two groups before treatment,the arterial partial pressure of oxygen(PaO2)and blood pH increased and the partial pressure of carbon dioxide(PaCO2)decreased after 2 weeks of treatment,and the improvement range in the experimental group was higher than that in the conventional group(P<0.05).After 2 weeks of treatment,forced expiratory volume in 1 second(FEV1),FEV1/forced vital capacity(FEV1/FVC),the maximum expiratory flow rate(PEF)increased in the experimental group(P<0.05),serum interleukin-8(IL-8),tumor necrosis factor-α(TNF-α),complement 1q/tumor necrosis factor-related protein 5(CTRP5),The level of chemokine ligand 12(CXCL12)showed a downward trend,but the decrease in the experimental group were more than those in the conventional group(P<0.05).There was no significant difference in adverse reactions between the two groups(P>0.05).Conclusion Methylprednisolone sodium succinate combined with tiotropium bromide can effectively improve blood gas indexes,lung function,and reduce lung inflammation in patients with AECOPD.
