A Clinical Study on Joint Detection of CYP2C19 Gene Polymorphism,Interleukin-6,D-Dimer and Fibrinogen in Patients with Acute Cerebral Infarction
To study the clinical value of joint detection of cytochrome P2C19(CYP2C19)gene polymorphism,interleukin-6(IL-6),D-dimer(D-D)and fibrinogen(FIB)in patients with acute cerebral infarction(ACI),120 ACI patients treated with clopi-dogrel bisulfate for 1 month at Dongguan Hospital of Integrated Traditional Chinese and Western Medicine in Guangdong Province were enrolled from November 2021 to September 2023.CYP2C19 genotypes were determined by polymerase chain reaction and microarray hybridization.According to genotypes,the subjects were divided into fast metabolism group,intermediate metabolism group and slow metabolism group.The correlation between CYP2C19 and the levels of IL-6,D-D and FIB was analyzed.The 3 groups were compared on the levels of IL-6,D-D and FIB before and after treatment,the National Institutes of Health Stroke Scale(NIHSS)scores and the prognosis.The significance of joint detection of CYP2C19 gene polymorphism and laboratory indicators was analyzed.It was found that the levels of IL-6,D-D and FIB in the slow metabolism group were higher than those in the intermediate metabolism group and the fast metabolism group(P<0.05).Correlation analysis found that CYP2C19 genotype was positively correlated with serum IL-6,D-D and FIB levels in patients with ACI(P<0.05).The levels of IL-6,D-D and FIB in the fast metabolism group were lower than those in the intermediate metabolism group and the slow metabolism group after treatment(P<0.05).The poor prognosis rate in the fast metabolism group[29.51%(18/61)]was lower than that in the intermediate metabolism group[50.00%(15/30)]and that in the slow metabolism group[62.07%(18/29)],and there was a statistically significant difference between the fast metabolism group and the slow metabolism group(P<0.05).The ROC curve analysis found that the area under the curve,sensitivity and specificity of slow-metabolism CYP2C19 genotype combined with IL-6,D-D and FIB for predicting poor prognosis of patients with ACI were 0.897,84.27%and 82.53%,which were higher than those of prediction with single indicator(P<0.05).As stated above,CYP2C19 gene polymorphism is positively correlated with the levels of IL-6,D-D and FIB in patients with ACI.Joint detection of these indicators is helpful for prognosis.
