水飞蓟宾调节肠道菌群保护NAFLD患者肠屏障的机制研究

探究水飞蓟宾通过调节肠道菌群来保护非酒精性脂肪肝(NAFLD)患者肠屏障的作用机制,选取2022年5月-2023年5月期间医院收治的经临床诊断确诊的NAFLD患者共96例,以单号双号将其分为对照组和观察组.对照组以常规治疗为基础加用双环醇,观察组则在对照组的基础上加入水飞蓟宾治疗.比较两组患者临床疗效、肝功能、炎症因子水平、肠屏障功能、肠道菌群,以及统计不良反应情况.在干预后,观察组治疗总有效率47例(97.92%),与对照组40(83.33%)相比明显较高;观察组谷草转氨酶(AST)、丙氨酸转氨酶(ALT)和谷氨酰转肽酶(GGT)、血清白介素-6(IL-6)、血清白介素-8(IL-8)、肿瘤坏死因子-α(TNF-α)、脂多糖(LPS)、血清二胺氧化酶(DAO)、D-乳酸(D-LA)等指标均得到了较大程度的改善(P<0.05);α多样性结果显示,与对照组相比,观察组Chao 1指数(Chao 1)明显低于对照组;而Shannon多样性指数(Shannon index)则显著高于对照组(P<0.05);观察组肠道菌群双歧杆菌属、乳酸菌明显高于对照组,而大肠杆菌相较于对照组则较低(P<0.05).不良反应的发生率方面,观察组显著低于对照组.在NAFLD患者中应用水飞蓟宾具有较高的治疗价值,对患者的肝功能具有较好的改善作用,并且能够对肠屏障起到保护作用,同时在一定程度上能降低患者的不良反应发生情况.
Mechanism of Silymarin Regulating Intestinal Flora to Protect the Intes-tinal Barrier of NAFLD Patients
To investigate the mechanism of action of silymarin in protecting the intestinal barrier in patients with non-alcoholic fatty liver disease by modulating intestinal flora,a total of 96 cases of NAFLD patients admitted to the hospital during the period of May 2022 to May 2023 who were confirmed by clinical diagnosis were selected,and they were divided into the control group and the observation group by single number and double number.The control group was treated with dicyclomine on the basis of conventional treatment,while the observation group was treated with silymarin on the basis of the control group.The clinical efficacy,liver func-tion,inflammatory factor level,intestinal barrier function,intestinal flora,and statistical adverse reactions were compared between the 2 groups.After the intervention,the total effective rate of treatment in the observation group was significantly higher in 47 cases(97.92%)compared with 40 cases(83.33%)in the control group;the levels of glutamic transaminase(AST),alanine amin-otransferase(ALT),glutamyltranspeptidase(GGT),serum interleukin-6(IL-6),serum interleukin-8(IL-8),tumour necrosis factor-α(TNF-α),lipopolysaccharide(LPS),serum diamine oxidase(DAO)and D-lactic acid(D-LA),were improved to a greater extent in the observation group(P<0.05).α-diversity results showed,compared with the control group,the Chao 1 index(Chao 1)of the observation group was significantly lower than that of the control group,while the Shannon diversity index(Shannon index)was significantly higher than that of the control group(P<0.05);the intestinal flora of Bifidobacterium spp.and Lactobacillus spp.in the observation group was significantly higher than that of the control group,whereas Escherichia coli was lower in comparison with the control group(P<0.05).Regarding the incidence of adverse reactions,the observation group was significantly lower than the control group.The application of silymarin in patients with NAFLD has high therapeutic value and a better effect on the improvement of liver function.It can play a protective effect on the intestinal barrier,and can reduce the occurrence of adverse reactions in patients.
