摘要

目的 分析视网膜静脉阻塞与血脂代谢、凝血功能及颈内动脉弹性功能参数之间的关系.方法 前瞻性队列研究.纳入濮阳市眼科医院2021年7月至2023年3月收治的视网膜静脉阻塞患者65例(65只眼)作为观察组,另随机连续收集60例(60只眼)体检为健康者作为对照组.比较两组血脂代谢、凝血功能指标及颈内动脉弹性功能参数等.结果 观察组患者的总胆固醇(TC)[(5.41±1.09)mmol/L]、甘油三酯(TG)[(2.18±1.24)mmol/L]以及低密度脂蛋白胆固醇(LDL-C)[(3.13±0.65)mmol/L]水平均显著高于对照组[(4.43±1.11)、(1.37±0.91)、(2.63±0.73)mmol/L],差异均有统计学意义(t=4.98、4.14、4.05,均P＜0.001);观察组患者高密度脂蛋白胆固醇(HDL-C)为(1.07±0.24)mmol/L,显著低于对照组的(1.32±0.26)mmol/L(t=5.59,P＜0.001).观察组患者凝血指标凝血酶原时间(PT)[(10.07±1.32)s]、活化部分凝血活酶时间(APTT)[(21.75±15.39)s]及凝血酶时间(TT)[(15.83±1.32)s],均明显低于对照组[(11.07±1.08)、(30.45±5.14)、(12.96±1.66)s],差异具有统计学意义(t=4.61、4.17、10.74,均P＜0.001);观察组纤维蛋白原(FIB)为(6.99±2.77)g/L,高于对照组的(3.30±1.69)g/L(t=9.90,P＜0.001).观察组中患者的颈内动脉紧张度为7.14±1.36、动脉扩张性[(2.43±0.24)kPa-1]以及顺应性[(0.38±0.11)mm2·kPa-1],低于对照组的 9.15±2.62、(2.65±0.48)kPa-1、(0.44±0.15)mm2·kPa1,差异具有统计学意义(t=5.44、3.28、2.56,均 P＜0.05).观察组中患者的颈内动脉紧张度(7.14±1.36)、动脉扩张性[(2.43±0.24)kPa-1]以及顺应性[(0.38±0.11)mm2·kPa-1]低于对照组[9.15±2.62、(2.65±0.48)kPa-1、(0.44±0.15)mm2·kPa-1],差异具有统计学意义(t=5.44、3.28、2.56,均P＜0.05);观察组患者压力-应变性系数为(127.38± 14.14)kPa,高于对照组的(107.38±10.29)kPa,差异具有统计学意义(t=8.98,P＜0.001).结论 与健康人群相比,视网膜静脉阻塞患者的血脂代谢指标(TC、TG、LDL-C、HDL-C)、凝血功能指标(PT、APTT、TT、FIB)及颈内动脉弹性功能指标均异常,且各项指标与视网膜静脉阻塞病变密切联系.

Abstract

Objective To analyze the relationship between retinal vein occlusion and blood lipid metabolism,coagulation function and internal carotid artery elastic function parameters of internal carotid artery.Methods This was a prospective cohort study.Sixty-five eyes of 65 cases of retinal vein occlusion treated in Puyang Eye Hospital from Jul.2021 to Mar.2023 were included as research subjects in the observation group.In addition,60 cases(60 eyes)were randomly selected as healthy subjects in the control group.The differences of blood lipid metabolism,coagulation function and internal carotid artery elastic function between the two groups were measured and recorded.Results The levels of total cholesterol(TC)[(5.41±1.09)mmol/L],triglyceride(TG)[(2.18±1.24)mmol/L],and low density lipoprotein cholestasis(LDL-C)[(3.13±0.65)mmol/L]in the observation group were significantly higher than those in the control group[(4.43±1.11),(1.37±0.91),(2.63±0.73)mmol/L],and the differences were statistically significant(t=4.98,4.14,4.05;all P＜0.001).The level of high density lipoprotein cholesterol(HDL-C)[(1.07±0.24)mmol/L]in the observation group was significantly lower than that in the control group[(1.32±0.26)mmol/L](t=5.59,P＜0.001).Prothrombin time(PT)[(10.07±1.32)s],activated partial thromboplastin time(APTT)[(21.75±15.39)s]and thrombin time(TT)[(15.83± 1.32)s]in the observation group were significantly lower than those in the control group[(11.07±1.08),(30.45±5.14),(12.96±1.66)s],and the differences were statistically significant(t=4.61,4.17,10.74;all P＜0.001).The level of fibrinogen(FIB)[(6.99±2.77)g/L]was higher than that in the control group[(3.30±1.69)g/L](t=9.90,P＜0.001).The internal carotid artery tension(7.14±1.36),arterial distensibility[(2.43±0.24)kPa-1]and compliance[(0.38±0.11)mm2·kPa-1]in the observation group were lower than those in the control group[9.15±2.62,(2.65±0.48)kPa-1,(0.44±0.15)mm2·kPa-1],and the differences were statistically significant(t=5.44,3.28,2.56;all P＜0.05).The pressure-strain coefficiency of the observation group[(127.38±14.14)kPa]was higher than that of the control group[(107.38±10.29)kPa],and the difference was statistically significant between the two groups(t=8.98,P＜0.001).Conclusion The blood lipid metabolism indexes(TC,TG,LDL-C,HDL-C),coagulation function(PT,APTT,TT,FIB)and elastic function of internal carotid artery are abnormal in patients with retinal vein occlusion,and all indexes are closely related to retinal vein occlusion.
