早期严重眼化学烧伤应用湿性羊膜覆盖的效果

摘要
目的 评估早期Ⅲ级及以上严重眼化学烧伤应用湿性羊膜覆盖的效果.方法 回顾性队列研究.收集福建医科大学附属三明第一医院2017年1月至2023年9月35例(35眼)Ⅲ级及以上早期眼化学伤患者的临床资料,根据不同治疗方法分为两组:对照组15例,常规药物治疗;观察组20例,在常规药物治疗的基础上,联合湿性羊膜覆盖.治疗后观察眼表稳定情况、上皮愈合情况、新生血管及视力等.随访时间3~24个月.结果 观察组眼表修复时间短于对照组(x2=9.48,P=0.024),角膜修复后视力高于对照组(Z=2.76,P=0.006),观察组并发症统计发生率(32.85%,46/140)少于对照组(59.04%,62/105)(x2=13.17,P=0.032),差异均具有统计学意义(均P<0.05).结论 早期Ⅲ级及以上严重眼化学烧伤应用湿性羊膜覆盖治疗效果良好.
Abstract
Objective To evaluate the efficacy of wet amniotic membrane covering for severe ocular chemical injury of grade Ⅲ or above in the early stage of the eye.Methods This was a retrospective cohort study.The clinical data of 35 eyes of 35 patients with grade Ⅲ or above early ocular chemical injuries Jan.2017 to Sep.2023 in Sanming First Hospital Affiliated to Fujian Medical University were collected.Based on different treatment methods,they were divided into two groups:the conventional drug treatment group of 15 cases was set as the control group,and the observation group of 20 cases received conventional drug treatment combined with wet amniotic membrane covering.The results of the stability of the ocular surface,epithelial healing,neovascularization,vision,etc were observed and followed up after treatment.The follow-up time was 3-24 months.Results The observation group had a shorter time for ocular surface repair compared to the control group(x2=9.48,P=0.024),and visual acuity after corneal repair was higher than that of the control group(Z=2.76,P=0.006).The incidence of complications in the observation group(32.85%,46/140)was lower than the 59.04%(62/105)in the control group(x2=13.17,P=0.032).All the differences mentioned above were statistically significant(all P<0.05).Conclusion The treatment effect of early grade Ⅲ and above severe ocular chemical wet amniotic membrane is good.
