应用OPD-Scan Ⅲ测量角膜横径的准确性和可重复性研究

摘要
目的 评估应用OPD-Scan Ⅲ测量角膜横径的可重复性,并比较其与Pentacam测量角膜横径的一致性.方法 横断面研究.纳入2023年2月至5月于苏州大学理想眼科医院就诊的近视者158例(316眼),分别使用OPD-Scan Ⅲ、Pentacam连续测量其角膜参数3次,记录其角膜横径(WTW)数值.使用重复测量方差分析检验同一仪器重复测量的组内差异;使用配对t检验比较两仪器测量均值的组间差异;使用Pearson相关性分析两种设备测量的角膜横径均值间相关性,并运用线性回归分析其影响关系;使用变异系数(CV)、克隆巴赫系数(CoA)及组内相关系数(ICC)评价OPD-Scan Ⅲ测量角膜横径的可重复性;绘制Bland-Altman图表法分析两设备的一致性.结果 OPD-ScanⅢ重复测量角膜横径的3次数据间差异有统计学意义(F=45.04,P<0.001),Pentacam重复测量角膜横径的3次数据间差异无统计学意义(F=0.72,P>0.05),两仪器测量角膜横径均值的差异存在统计学意义(t=35.85,P<0.001);OPD-Scan Ⅲ与Pentacam两仪器测量角膜横径的CV值分别为3.30%、3.20%,CoA 为 0.994、0.997,ICC 为 0.978、0.990;OPD-Scan Ⅲ与 Pentacam 测量角膜横径的一致性较差,95%LOA 为 0.01~0.59 mm,95%LOA 线外点比例为 4.43%;OPD-Scan Ⅲ、Pentacam 测量的角膜横径间存在高度正相关性(r=0.93,P<0.001);OPD-Scan Ⅲ与Pentacam测量角膜横径间存在线性关系,回归方程为WTWO=0.386+0.992 WTWP(调整后R2=0.86,P<0.001).结论 OPD-Scan Ⅲ测量角膜横径的可重复性较好,但与Pentacam相比测量角膜横径的一致性较差.
Abstract
Objective To evaluate the repeatability of measuring corneal transverse diameter with OPD-Scan Ⅲ and to compare the agreement between OPD-Scan Ⅲ and Pentacam.Methods This was a cross-sectional study.A total of 316 eyes of 158 myopia patients admitted to Lixiang Eye Hospital of Soochow University from Feb.2023 to May 2023 were included.Corneal parameters were measured with OPD-ScanⅢ and Pentacam for 3 consecutive times,and corneal transverse diameter(white to white,WTW)was recorded.Intra-group differences were analyzed by repeated ANOVA of the same instrument.Inter-group differences were analyzed by paired t-test to compare the mean vaule of WTW.Pearson correlation analysis was employed to analyze the correlationship between the WTW values measured with the two devices,while the linear regression was employed to assess the influence relationship.The coefficient of variation(CV),Cronbach's coefficient alpha(CoA)and interclass correlation coefficient(ICC)were calculated to evaluate the repeatability.Bland-Altman plots were drawn to analyze the agreement of the two devices.Results The differences among the three-time repeated measurements of OPD-Scan Ⅲ and Pentacam showed different significance of repeated ANOVA(F=45.04,P<0.001;F=0.72,P>0.05),respectively.The difference of WTW values measured averagely with the two instruments was statistically significant(t=35.85,P<0.001).The CV value,CoA value,ICC value of OPD-Scan Ⅲ and Pentacam were(3.3%,3.2%),(0.994,0.997),(0.978,0.990),respectively.The 95%limits of agreement(LOA)were 0.01-0.59 mm,and the percentage of points outside the 95%LOA line reached 4.43%.The WTW values measured by OPD-Scan Ⅲ and Pentacam showed high positive correlation(r=0.93,P<0.001),and showed linear relationship with a proposed regression equation as WTWO=0.386+0.992 WTWP(adjusted R2=0.86,P<0.001).Conclusion OPD-Scan Ⅲ perform good repeatability in measuring WTW,but the agreement is too poor,compared to Pentacam.
