高度近视者有晶状体眼后房型人工晶状体植入术后拱高的临床研究

摘要
目的 观察高度近视者行有晶状体眼后房型人工晶状体(ICL)植入术后拱高及其影响因素.方法 回顾性病例系列研究.分析徐州医科大学附属徐州市第一人民医院 2019 年 1 月至2020 年 11 月进行ICL 植入术治疗高度近视者共 18 例(26 眼)的临床资料,通过 Pentacam眼前节分析仪观察术后拱高,分析白到白距离、前房深度等因素对于术后拱高的影响,对上述观察指标进行Pearson相关性分析.术后随访12个月.结果 所有患者术后3个月拱高[(190.00±45.26)μm]较术后 1个月[(198.85±44.84)μm]降低,差异有统计学意义(t=8.85,P=0.046),术后 6 个月拱高[(180.19±48.22)μm]较术后 3 个月降低,差异有统计学意义(t=18.65,P=0.003),术后 12 个月拱高[(176.15±45.26)μm]较术后 6 个月差异无统计学意义(t=22.69,P=0.098).通过Pearson相关性分析发现,术前及对应时间点前房深度与术后拱高呈显著正相关(均P<0.05).结论 高度近视者行ICL植入术后拱高随时间逐渐降低,术前前房深度是术后拱高的影响因素.
Abstract
Objective To observe the vault and its influencing factors in patients with high myopia after phakic posterior chamber implantable collamer lens(ICL)implantation.Methods This was a retrospective case series study.Clinical data of 26 eyes of 18 patients with high myopia treated with ICL implantation from Jan.2019 to Nov.2020 in Xuzhou First People's Hospital Affiliated to Xuzhou Medical University were analyzed.The vault after surgery was observed by Pentacam anterior segment analysis system.The effects of white to white distance and anterior chamber depth on postoperative vault were analyzed,and Pearson correlation analysis was performed for the above observation indicators.The follow-up time was 12 months after surgery.Results By measuring the vault at different time points,it was found that the vault at 3 months[(190.00±45.26)μm]after surgery was significantly lower than that at 1 month[(198.85±44.84)μm]after surgery(t=8.85,P=0.046);the vault at 6 months[(180.19±48.22)μm]after surgery was significantly lower than that at 3 months after surgery(t=18.65,P=0.003);the vault at 12 months[(176.15±45.26)μm]after surgery was not significantly different from that at 6 months after surgery(t = 22.69,P= 0.098).According to Pearson correlation analysis,anterior chamber depth and postoperative vault were significantly positively correlated before and at corresponding time points(all P<0.05).Conclusion After ICL implantation,the vault of high myopia gradually decreases with time,and the preoperative anterior chamber depth is the influencing factor of the vault.
