高度近视合并白内障患者人工晶状体屈光度计算公式的准确性

摘要
目的 研究SRK/T、Barrett Ⅱ与Kane 3 种公式计算高度近视合并白内障患者(屈光度-6.00 D以上)所需人工晶状体屈光度的准确性.方法 回顾性病例系列研究.纳入 2023 年 6 月至2023 年 12 月在开封市中心医院眼科行白内障超声乳化吸除人工晶状体植入术的高度近视合并白内障患者 78 例(78 眼).根据眼轴长度(AL)患者分为 3 组:A组,26.00 mm≤AL<28.00 mm;B组,28.00 mm≤AL<30.00 mm;C组,AL≥30.00 mm.术前行IOL Master700 和各项常规检查,分别应用SRK/T、Barrett Ⅱ、Kane在线公式计算所需人工晶状体屈光度,术后 1 个月行屈光度检查,计算比较其平均预测误差(ME)和绝对值误差中位数(MedAE).结果 三组患者每个公式的ME与 0 比较差异有统计学意义(均P<0.05),Friedman检验显示B、C组公式MedAE差异有统计学意义(χ2=12.28、15.15,P=0.031、0.023),A组MedAE差异无统计学意义(χ2=6.47,P=0.142).组内ME两两比较均显示Kane公式与Barrett Ⅱ公式差异无统计学意义(均P>0.05);SRK/T公式与其他公式比较差异有统计学意义(均P<0.05).三组不同公式ME在±0.50 D和±1.0 D范围内的比率差异均有统计学意义(均P<0.05).结论 Kane和Barrett Ⅱ公式在白内障患者合并高度近视超长眼轴情况下均有着较好的屈光预测准确性,优于SRK/T公式.
Abstract
Objective To evaluate the accuracy of SRK/T,Barrett Ⅱ,and Kane formulas in calculating the refractive power of intraocular lenses for cataract patients with high myopia(diopter above-6.00 D).Methods This was a retrospective case series study.A total of 78 eyes of 78 cases with cataract and high myopia were includeded in Kaifeng Central Hospital from Jun.2023 to Dec.2023.All cases received phacoemulsification combined with IOL implantation.Patients were categorized into three groups based on their axial length(AL):group A,26.00 mm≤AL<28.00 mm;group B,28.00 mm≤AL<30.00 mm;group C,AL≥30.00 mm.Before surgery,IOL Master700 and various routine examinations were performed.The IOL power was calculated using the SRK/T,Barrett Ⅱ,and Kane online formulas,respectively.At one month after surgery,subjective optometry was examined and the mean error(ME)and median absolute error(MedAE)were calculated and compared.Results There was a significant difference in ME between each formula and 0 in the three groups of patients(all P<0.05).Friedman's test revealed the existence of a notable disparity in MedAE in group B and group C(χ2=12.28,15.15;P=0.031,0.023),but no significant difference in group A(χ2=6.47,P=0.142).The comparison of ME of the three groups showed no significant difference between the Kane formula and the Barrett Ⅱ formula(all P>0.05).The difference between the SRK/T formula and other formulas was statistically significant(all P<0.05).The differences in the ratios of the three different formulas within the range of ME±0.50 D and±1.0 D were statistically significant(all P<0.05).Conclusion Both Kane and Barrett Ⅱ formulas have good refractive prediction accuracy in patients with cataract and high myopia,which is superior to the SRK/T formula.
