内镜下经鼻腔入路植入可吸收固定材料治疗眶内壁骨折的临床效果

摘要
目的 探讨内镜下经鼻腔入路植入可吸收固定材料治疗眶内壁骨折的效果.方法 回顾性病例系列研究.纳入 2021 年 1 月至 2022 年 12 月于山东省立第三医院确诊为单眼眼眶内壁骨折且眼球内陷≥2.00 mm的 16 例患者,进行内镜下经鼻腔入路植入可吸收固定材料修复眶内壁骨折手术.术后随访 1 周至半年,观察眼球突出度变化和眼球运动等情况.结果 术前眼球突出度为(12.94±0.82)mm,术后半年为(15.56±0.79)mm,差异有统计学意义(t=-18.66,P=0.001).随访期间显效 13 例,有效 3 例.16 例均无复视、眼球运动受限及明显并发症.结论 内镜下经鼻腔入路植入可吸收固定材料治疗眶内壁骨折可以有效减轻或消除因眶内壁骨折造成的眼球内陷.
Abstract
Objective To analyze the efficacy of surgeries for intraorbital wall fracture,by implanting absorbable fixing materials through endoscopic transnasal approach.Methods This was a retrospective case series study.Sixteen cases from Jan.2021 to Dec.2022 diagnosed as single medial orbital fracture with enophthalmos greater than 2.00 mm were selected in Shandong Provincial Third Hospital.All patients underwent surgeries for intraorbital wall fracture,with absorbable fixing materials implanted through endoscopic transnasal approach.The changes of exophthalmos and eye movements,etc.were observed in the follow-up after surgery from 1 week to half a year.Results The average value of preoperative eyeball protrusion was(12.94±0.82)mm.After the follow-up for half a year,the average value of eyeball protrusion was(15.56±0.79)mm.Eyeball protrusion markedly increased after the surgeries.The difference was statistically significant(t=-18.66,P=0.001).It was significantly effective in 13 cases and effective in 3 cases during the follow-up time.None of the 16 cases had diplopia,limited eye movement,or obvious complications.Conclusion Surgeries for intraorbital wall fracture by implanting absorbable fixing materials through endoscopic transnasal approach can effectively alleviate,even eliminate the enophthalmos caused by the fracture of the inner wall of the orbit.
