迟发性眼前节毒性反应综合征一例
A case of delayed onset toxic anterior segment syndrome

摘要
患者,女性,63 岁.因左眼原发性急性闭角型青光眼行白内障超声乳化人工晶状体植入联合房角分离术.术后1 d裸眼视力0.8,前房炎症反应轻微.术后14 d视力下降至0.1,并伴混合充血和轻微疼痛、角膜上皮片状剥脱、角膜后尘状沉积物、前房纤维渗出、人工晶状体表面覆盖致密膜状物.眼科B超检查玻璃体无明显浑浊.为了与感染性眼内炎鉴别,抽取房水检查,结果未检测到致病微生物,但白介素-6 和白介素-8 显著升高,提示眼内炎症活动严重.经过3 周局部和全身使用皮质类固醇治疗,角膜恢复透明,前房和人工晶状体上的膜状物消失,裸眼视力提高至0.5,矫正视力 1.0.
Abstract
The patient was a 63-year-old female.Phacoemulsification and intraocular lens implantation combined with goniosynechialysis were performed for primary acute angle-closure glaucoma in the left eye.At 1 day after surgery,the naked-eye visual acuity was 0.8,and the anterior chamber inflammation was mild.Visual acuity decreased to 0.1 at 14 days after surgery,accompanied by mixed congestion and mild pain,lamellar exfoliation of the corneal epithelium,posterior corneal dust deposits,anterior chamber fiber exudation,and a dense membrane covering the intraocular lens.There was no obvious turbidity in the vitreous body by B-mode ultrasonography.In order to distinguish it from infective endophthalmitis,aqueous humor was extracted.No pathogenic micro-organism was detected,but interleukin-6 and interleukin-8 significantly increased,suggesting serious intraocular inflammation.After 3 weeks of local and systemic corticosteroid treatment,the cornea returned to transparency;the membrane on the anterior chamber and intraocular lens disappeared;and the naked-eye visual acuity improved to 0.5 and the corrected visual acuity imporved to 1.0.
