摘要

目的:探讨声触诊组织成像技术(VTI)在鉴别TI-RADS4类甲状腺实性结节良恶性中的应用价值.方法:2020年10月-2021年10月,我院甲乳外科收治的92例患者105个经常规超声(conventional ultrasonography,CUS)诊断为TI-RADS4类的甲状腺结节.弹性检查:行VTI成像,获取VTI弹性图,找出VTI面积比(AR)鉴别TI-RADS4类甲状腺实性结节良恶性的最佳截点,以病理诊断为金标准,分析CUS及VTI技术诊断TI-RADS4类甲状腺实性结节的灵敏度、特异度和准确度.结果:以AR=1.28作为诊断TI-RADS 4类甲状腺恶性结节的最佳截点,灵敏度为81.7%,特异度为92.1%,准确度为86.3%.结论:CUS联合VTI技术诊断TI-RADS4类甲状腺实性结节的准确度明显增高.

Abstract

Objective:To investigate the application value of virtual touch tissue imaging(VTI)in the differentiation of benign and malignant TI-RADS4 solid thyroid nodules.Methods:From October 2020 to October 2021,105 thyroid nodules in 92 patients admitted to the Department of Surgery in our hospital were diagnosed as TI-RADS4 by conventional ultrasonography(CUS).Elasticity test:VTI were per-formed,the VTI elastic map were obtained.VTI area ratio(AR)was found to identify the best cut-off points for the identification of benign and malignant TI-RADS4 solid thyroid nodules.Using pathological diagnosis as the gold standard,the sensitivity,specificity and accuracy of CUS and VTI techniques in di-agnosing TI-RADS4 thyroid nodules were analyzed.Results:AR=1.28 was used as the best cut-off point for the diagnosis of the four types of thyroid malignant nodules with TI-RADS.The sensitivity,specificity and accuracy were 81.7%,92.1%and 86.3%.Conclusion:CUS combined with VTI technology can signifi-cantly improve the accuracy of diagnosing TI-RADS4 thyroid nodules.
