中国不同地区儿科麻醉学专科医师规范化培训现状分析
Current status of standardized training of pediatric anesthesiologists in different regions of China

摘要
目的 了解我国不同地区儿科麻醉学专科医师规范化培训(简称"专培")的现状,以及专培医师对培训过程和结果的反馈情况.方法 在这项横断面研究中,基于2022年全国调查的数据,对我国39个儿科麻醉专培基地已完成专培的麻醉医师进行调查.根据国家统计局的报告,将我国划分为4个区域进行分析.统计学分析软件采用IBM SPSS Statistics 25,对于多组间的计量资料及等级资料,均采用Kruskal-Wallis H检验.结果 共回收174份调查问卷,有效回收问卷155份(89.08%).我国参加专培的医师人群主要以31~35岁的已婚女性为主,她们多为硕士学位,在专培前工作于综合性三甲医院,且注册专业为外科.在专培保障方面,东部地区的工作时长明显低于中部地区(P=0.033),而中部和东部地区的时薪则明显高于东北部地区(P=0.005).在专培医师对专培考核和管理制度的满意度方面,西部地区在招收时的临床综合能力考核方面及结业理论考试的系统操作方面的医师满意度均明显高于东北部地区(P=0.017,P=0.031).在专培医师对专培教学活动完成情况的满意度方面,东北部地区在360度评估方面的完成度明显高于东部地区(P=0.015),西部地区在保证每周1次教学活动方面的表现明显优于东北部地区(P=0.003).此外,东北部地区在独立进行门诊工作方面的表现也明显高于中部地区(P=0.034),而西部地区在结业理论考核的难易度与专培目标的一致性方面则明显高于东北部地区(P=0.046).在专培医师自我评价方面,西部地区的临床思维评价明显高于东部地区(P=0.011).结论 我国不同地区在专培医师保障、专培管理制度及教学内容的设置,以及专培医师的自我评价方面均存在一定的差异.根据此次反馈,可以促进地区间进行交流学习,共同提高专培的质量,从而提升专培医师的满意度.
Abstract
Objective To investigate the current status of standardized training of pediatric anesthesiologists in different regions of China and the feedback from trainees on the training process and results.Methods In this retrospective cross-sectional study based on the data of the national survey in 2022,a questionnaire survey was conducted among the anesthesiologists who had received specialized training in 39 training bases for pediatric anesthesia in China,and based on the report of National Bureau of Statistics,China was divided into four regions for analytical purposes.IBM SPSS Statistics 25 was used to perform statistical analyses,and the Kruskal-Wallis H test was used for comparison of continuous data and ranked data between multiple groups.Results A total of 174 questionnaires were collected,among which there were 155 valid questionnaires(89.08%).Most individuals participating in the specialized training were married women aged 31-35 years,and they mainly held a master's degree and worked at a comprehensive grade A tertiary hospital before training,with a registered specialty of surgery.As for the supporting measures for specialized training,the working hours in the eastern region were significantly shorter than those in the central region(P=0.033),while the hourly wage in both the central and eastern regions was significantly higher than that in the northeast region(P=0.005).As for the degree of satisfaction with the assessment and management systems of the specialized training,compared with the northeastern region,the western region showed a significantly higher degree of satisfaction with the assessment of comprehensive clinical ability at the time of recruitment and the system operation of final theoretical assessment(P=0.017 and 0.031).As for the degree of satisfaction with the completion of teaching activities of the specialized training,the northeast region showed a significantly higher degree of completion of 360-degree evaluation than the eastern region(P=0.015),while compared with the northeast region,the western region showed significantly better performance in guaranteeing the teaching activities once a week(P=0.003).In addition,the northeast region showed significantly better performance than the central region in terms of independent outpatient services(P=0.034),while the western region showed significantly better results than the northeast region in the difficulty of final theoretical assessment and its consistency with the goals of specialized training(P=0.046).As for the self-evaluation of physicians receiving specialized training,the western region showed a significantly higher self-evaluation of clinical thinking than the eastern region(P=0.011).Conclusions There are certain differences in the supporting measures,management systems,and teaching contents of specialized training and self-evaluation by physicians receiving specialized training.This feedback can enhance the exchange and learning across different regions of China,promote the quality of specialized training,and improve the degree of satisfaction among physicians receiving specialized training.
