Predictive Value of GNRI Combined with GRACE Score for the Severity of Coronary Artery Disease in Elderly Patients with Acute Coronary Syndrome
Objective To investigate the predictive value of GNRI combined with GRACE score for the severity of coronary artery disease in elderly patients with ACS.Methods A total of 222 patients with ACS who underwent selective coronary angiography in the Department of Cardiology, the First Affiliated Hospital of Anhui Medical University from July to December 2022 due to chest pain were retrospectively analyzed.According to the SYNTAX score, the patients were divided into low-risk group(170 patients),medium-risk group(34 patients)and high-risk group(18 patients).Logistic regression was used to analyze the risk factors of severe coronary artery disease in elderly ACS patients.Spearman correlation analysis was used to analyze the correlation between GNRI and GRACE scores with SYNTAX score, coronary artery stenosis score and coronary artery lesion characteristic score, respectively. The receiver operating characteristic(ROC)curve was drawn to evaluate the predictive value of GNRI,GRACE score and their combination for severe coronary artery disease in elderly ACS patients. Results GNRI and GRACE scores were independently associated with severe coronary artery disease and were independent influencing factors of severe coronary artery disease(P<0.05).GNRI and GRACE scores were significantly correlated with SYNTAX score, coronary artery stenosis score and coronary artery lesion characteristic score(P<0.05).The area under the ROC curve drawn by the combined coefficient of GNRI and GRACE score was 0.817,the sensitivity and specificity were 83.30%and 75.50%,respectively,which could better predict the severity of coronary artery disease in elderly ACS patients. Conclusion The GNRI and GRACE scores of elderly ACS patients can reflect the degree of coronary artery disease to a certain extent. The combination of the two is helpful to evaluate the severity of coronary artery disease in elderly ACS patients, and provides a simple and reliable evaluation basis for screening high-risk ACS patients.
