Research on the Evaluation of the Effect of DIP Medical Insurance Payment Reform on the Cost Control of Primary Hospitals
Objective To evaluate the effect of diagnosis-intervention packet (DIP) medical insurance payment reform on cost control in primary hospitals.Methods From January to June 2023,a total of 1857 cases of DIP medical insurance payment in Guixi People's Hospital were set as the post-implementation group,and another 1860 patients who did not implement DIP medical insurance payment in our hospital from January to June 2022 were selected as the pre-implementation group. The total hospitalization cost,drug cost,material cost,inspection cost,inspection cost,comprehensive medical service cost,diagnosis on the front page of medical records and missed filling of surgery,average hospitalization days,medical service quality and service efficiency were compared between the two groups. The difference-in-difference method was used to compare the effect values of medical expenses and hospitalization expenses between the two groups.Results The total cost of hospitalization the drug cost,and inspection cost in the post-implementation group were lower than those in the pre-implementation group (P<0.05),but there was no significant difference in material cost,inspection cost and comprehensive medical service cost between the two groups (P>0.05). The diagnosis of the first page of the medical record and the missing filling of the operation in the post-implementation group were lower than those in the pre-implementation group,and the hospitalization days were shorter than those in the pre-implementation group (P<0.05). The scores of medical service quality and service efficiency in the post-implementation group were higher than those in the pre-implementation group (P<0.05). The effect values of medical expenses and hospitalization expenses in the post-implementation group were higher than those in the pre-implementation group (P<0.05).Conclusion The reform of DIP medical insurance payment method has a high effect on the control of fees in primary hospitals,which is mainly reflected in drug costs and inspection costs. At the same time,it can shorten the number of hospitalization days to a certain extent,improve the quality and efficiency of medical services,and reduce the diagnosis of the first page of medical records and the missed filling of surgery. However,there are few pilot projects at present,and the specific feasibility needs to be further studied and confirmed.
