目的 系统评价黄芪桂枝五物汤治疗腰椎间盘突出症(LDH)的临床疗效.方法 根据循证医学研究方法,在中国知网、维普网、万方数据库、中国生物医学文献服务系统、Web of Science、PubMed及Cochrane Library中检索关于黄芪桂枝五物汤治疗LDH的临床随机对照研究,检索时间为建库至2023年7月31日.检索采取主题词与自由词结合的方式,按照Cochrane操作手册独立进行文献纳入、筛选与排除、提取数据和偏倚风险评估,汇总整理后采用Review Manager 5.4软件进行Meta分析.结果 最终纳入10篇文献,涉及755例LDH患者.Meta分析结果示:黄芪桂枝五物汤治疗LDH在显效率[RR=1.25,95％CI(1.00,1.56),P=0.05]、优良率[RR=1.16,95％CI(1.05,1.27),P<0.01]、视觉模拟疼痛量表(VAS)评分[MD=-1.16,95％CI(-1.34,-0.98),P<0.00001]和腰椎Oswestry功能障碍指数(ODI)评分[MD=-4.69,95％CI(-5.09,-4.29),P<0.00001]等方面均优于对照组.结论 黄芪桂枝五物汤在腰椎间盘突出症的治疗中疗效确切,可提高临床疗效,缓解腰腿疼痛,改善临床症状.
Meta-analysis of Huangqi Guizhi Wuwu Decoction in the Treatment of Lumbar Disc Herniation
Objective To systematically evaluate the clinical efficacy of Huangqi Guizhi Wuwu decoction in the treatment of lumbar disc herniation (LDH).Methods According to the research method of evidence-based medicine,the clinical randomized controlled studies on Huangqi Guizhi Wuwu decoction in the treatment of LDH were searched in CNKI,VIP,Wanfang database,Chinese biomedical literature service system,Web of Science,PubMed and Cochrane Library. The retrieval time was from the establishment of the database to July 31,2023. The retrieval was carried out by combining subject words with free words. According to the Cochrane operation manual,literature inclusion,screening and exclusion,data extraction and bias risk assessment were carried out independently. Meta-analysis was performed using Review Manager 5.4 software. Results A total of 10 articles were included,involving 755 patients with LDH. The results of meta-analysis showed that Huangqi Guizhi Wuwu decoction was effective in the treatment of LDH was superior to the control group in terms of significant efficienc[RR=1.25,95％CI(1.00,1.56),P=0.05],excellent and good rate[RR=1.16,95％CI(1.05,1.27),P<0.01],visual analogue pain scale (VAS) score[MD=-1.16,95％CI(-1.34,-0.98),P<0.00001]and lumbar Oswestry dysfunction index (ODI) score[MD=-4.69,95％CI (-5.09,-4.29),P<0.00001].Conclusion Huangqi Guizhi Wuwu decoction is effective in the treatment of lumbar disc herniation,which can improve the clinical efficacy,relieve lumbocrural pain and improve clinical symptoms.
