Evaluate the Effect of Noninvasive Positive Pressure Ventilation on Cardiac Function in Patients with Acute Left Heart Failure of Coronary Heart Disease by Doppler Cardiac Function Index
Objective To study the value of Doppler cardiac function index (Tei index) in evaluating the effect of non-invasive positive pressure ventilation (NIPPV) on cardiac function in patients with coronary heart disease (CHD) and acute left heart failure (AHF). Methods A total of 66 patients with CHD and AHF admitted to Yudu County People's Hospital from January 2022 to December 2023 were selected as the research objects. All patients were treated with NIPPV. The therapeutic effects were statistically analyzed. The left ventricular ejection fraction (LVEF) before and after weaning was detected by echocardiography. At the same time,Doppler imaging mode was used to measure the ratio of early diastolic peak velocity to late diastolic peak velocity (E/A) and Tei index of left atrioventricular valve. The changes of LVEF,E/A and Tei index before and after weaning were compared. The evaluation value of different indicators on the improvement of cardiac function after NIPPV treatment was analyzed. Results After 2 hours of NIPPV ventilation treatment,a total of 62 patients reached the effective treatment standard,and the effective rate was 93.94%. The other 4 patients were ineffective and changed to tracheal intubation mechanical ventilation. After weaning,the LVEF and E/A parameters of patients with effective treatment were higher than those before treatment (P<0.05),and the Tei index was lower than that before treatment (P<0.05). The ROC curve showed that the value of Tei index in evaluating the improvement of cardiac function after NIPPV treatment was higher than that of LVEF and E/A (P<0.05).Conclusion Tei index can effectively reflect the effect of NIPPV on cardiac function in patients with coronary heart disease AHF,and has a positive evaluation value for the improvement of cardiac function.
