Clinical Value of CT in Differential Diagnosis of Papillary Thyroid Carcinoma and Nodular Goiter
Objective To study the clinical value of CT in differential diagnosis of papillary thyroid carcinoma and nodular goiter. Methods A total of 76 patients with thyroid diseases diagnosed and treated in our hospital from July 2021 to June 2023 were selected as the research objects,including 32 patients with papillary thyroid carcinoma and 44 patients with nodular goiter. All of them were diagnosed by CT. The diagnostic efficacy of CT in differential diagnosis of papillary thyroid carcinoma and nodular goiter (sensitivity,specificity,accuracy,positive predictive value,negative predictive value) was observed. The CT imaging characteristics and thyroid volume of papillary thyroid carcinoma and nodular goiter were compared. ROC curve was used to analyze the value of CT in differential diagnosis of papillary thyroid carcinoma and nodular goiter. Results There was no significant difference in the sensitivity,specificity,accuracy,positive predictive value and negative predictive value of CT in the differential diagnosis of papillary thyroid carcinoma and nodular goiter (P>0.05). There were significant differences between papillary thyroid carcinoma and nodular goiter in morphology,margin,internal characteristics,calcification,halo,and relationship with glands (P<0.05),while there was no significant difference in the types of nodules between papillary thyroid carcinoma and nodular goiter (P>0.05). The nodule volume and the thickness of the gland isthmus of papillary thyroid carcinoma were larger than those of nodular goiter,and the volume of the right lobe gland and the left lobe gland were smaller than those of nodular goiter (P<0.05). ROC curve showed that there was no significant difference in AUC area between CT diagnosis of papillary thyroid carcinoma and nodular goiter (P>0.05).Conclusion CT has a high value in the differential diagnosis of papillary thyroid carcinoma and nodular goiter,and it can be judged by CT imaging characteristics to understand the specific characteristics of the disease. It is of great significance for the follow-up treatment of patients and is worthy of clinical application.
