Value of Color Doppler Ultrasound in the Diagnosis of Lower Extremity Arteriosclerosis Obliterans and its Guiding Value for Clinical Treatment
Objective To investigate the value of color Doppler ultrasound in the diagnosis of lower extremity arteriosclerosis obliterans and its guiding value for clinical treatment. Methods A total of 56 patients with suspected lower extremity arteriosclerosis obliterans who were treated in our hospital from November 2021 to October 2024 were selected as subjects. All patients underwent color Doppler ultrasound and digital subtraction angiography. The value of color Doppler ultrasound in the detection of vascular lesions(mild stenosis, moderate stenosis, severe stenosis, occlusion)in patients with lower extremity arteriosclerosis obliterans was analyzed. The results of digital subtraction angiography were used as the gold standard to analyze the positive predictive value, negative predictive value, sensitivity, specificity and accuracy of color Doppler ultrasound in the diagnosis of lower extremity arteriosclerosis obliterans. Results Among 56 patients,47 patients were diagnosed by digital subtraction angiography.Color Doppler ultrasound diagnosis found 48 positive patients(46 true positive patients)in 56 patients, the positive predictive value was 95.83%,the negative predictive value was 87.50%,the sensitivity was 97.87%,the specificity was 77.78%,and the detection accuracy was 94.64%,there was no significant difference between the two methods(P>0.05).A total of 52 segments of stenosis/occlusion vessels were found in 47 patients by digital subtraction angiography, including mild stenosis(34.62%),moderate stenosis(28.85%),severe stenosis(21.15%)and occlusion(15.38%).A total of 50 segments of stenosis/occlusion blood vessels were found in 46 patients by color Doppler ultrasound diagnosis, including mild stenosis(38.00%),moderate stenosis(26.00%),severe stenosis(20.00%)and occlusion(16.00%),there was no significant difference between the two methods(P>0.05).Conclusion The accuracy of color Doppler ultrasound in the diagnosis of lower extremity arteriosclerosis obliterans is comparable to that of digital subtraction angiography. It can reduce missed diagnosis and misdiagnosis. It is a reliable and safe imaging method for the diagnosis of lower extremity arteriosclerosis obliterans.
