人工智能辅助识别IgA肾病和过敏性紫癜性肾炎的肾小球病理特征

目的 评估基于深度学习算法构建的肾脏病理分析系统(ARPS)在过敏性紫癜性肾炎(HSPN)患者中的应用效果,并分析在IgA肾病(IgAN)和HSPN中ARPS定量的肾小球特征与临床预后的关系.方法 回顾性分析2018年1月至2019年12月在国家肾脏疾病临床医学实验中心经肾活检确诊为IgAN(n=170)和HSPN(n=153)的患者资料.采用精确率、召回率和F1分数评估ARPS在HSPN患者中的识别效果,并应用ARPS识别其肾小球病变类型,如球性硬化(GS)、节段硬化(SS)、新月体(C)和非上述病变类型肾小球(NOA),并识别NOA肾小球中的固有细胞类型,包括系膜细胞(M)、内皮细胞(E)和足细胞(P)等.利用X-tile软件确定足细胞数字病理特征的最佳截止点,并将患者分为足细胞高值组(>最佳截止点)和足细胞低值组(≤ 最佳截止点).比较IgAN和HSPN患者的临床及病理特征,并分析患者数字病理特征与临床预后的关系.结果 ARPS对HSPN患者病变类型的识别效果较好,其中NOA、GS肾小球识别效果(F1分数分别为0.95和0.90)优于C和SS(F1分数分别为0.83和0.73).ARPS可准确定位HSPN患者的肾小球固有细胞(F1分数>0.91).两组患者的系膜细胞数字指标均与牛津分型系膜细胞增殖评分呈正相关(P<0.01).足细胞高值组患者的肾功能稳定率明显高于足细胞低值组(P<0.05).结论 ARPS可应用于HSPN患者的肾小球病变及固有细胞识别.系膜细胞数字指标可提示系膜细胞增殖程度,足细胞数字指标可辅助分析两种疾病的预后.
Artificial intelligence-assisted identification of glomerular pathological features in IgA nephropathy and Henoch-Sch?nlein purpura nephritis
Objective The study aims at evaluating the performance of the analytic renal pathology system(ARPS)based on deep learning in patients with Henoch-Schönleinpurpura nephritis(HSPN),and analyzing the relationship between the glomerular fea-tures quantified by the ARPS and clinical prognosis in patients with IgA nephropathy(IgAN)or HSPN.Methods The data of pa-tients diagnosed with IgAN(n=170)and HSPN(n=153)by renal biopsy in the National Clinical Experimental Center for Kidney Dis-eases from January 2018 to December 2019 were retrospectively analyzed.The precision,recall and F1 score were used to evaluate the recognition effect of ARPS in HSPN patients.ARPS was also applied to identify glomerular lesion types,such as glomerular scle-rosis(GS),segmental sclerosis(SS),crescents(C)and non-men-tioned glomerular lesions(NOA),and to identify the intrinsic cells in NOA glomeruli,including mesangial cells(M),endothelial cells(E)and podocytes(P).X-tile software was used to determine the optimal cutoff point of podocyte digital pathological features,and patients were divided into high podocyte value group(>optimal cut-off point)and low podocyte value group(≤ optimal cutoff point).The clinical and pathological features of IgAN and HSPN patients were compared,and the relationship between digital pathological features and clinical prognosis was analyzed.Results ARPS has a good effect on identifying the lesion types of HSPN patients,among which the recognition effects of NO A and GS glomeruli(F1 scores of 0.95 and 0.90,respectively)are better than those of C and SS(F1 scores of 0.83 and 0.73,respectively).ARPS can accurately locate the glomerular intrinsic cells in patients with HSPN(F1 score>0.91).The mesangial cell digital index of both groups of patients was positively correlated with the Oxford classification mesangial cell proliferation score(P<0.01).The renal function stability rate of pa-tients in the high podocyte value group was significantly higher than that in the low podocyte value group(P<0.05).Conclusion The ARPS can be applied to identify glomerular lesions and intrinsic cells in patients with HSPN.The mesangial cell digital index can indicate the degree of mesangial cell proliferation,and the podocyte digital index can assist in analyzing the prognosis of the two diseases.
