摘要
目的:调查上海市浦东新区三甲医院出院老年患者医院-家庭过渡期护理服务需求及支付意愿,为今后制定医院-家庭过渡期护理服务的收费、报销方式提供参考.方法:采用分层随机抽样法对上海市浦东新区三甲医院380 名出院老年患者进行问卷调查,统计描述医院-家庭过渡期护理服务需求和支付意愿、可接受的支付金额及报销比例,运用logistic回归对可接受的支付金额进行影响因素分析.结果:出院老年患者对医院-家庭过渡期护理服务需求高,大多(95.3%)有支付意愿,可接受的支付金额为 251.89 元/月、37.30元/项,可接受的报销比例为每月24.79%、每项33.75%.可接受的支付金额主要受到慢性病患病情况、医疗资源空间距离、家庭对医疗决策的影响程度等因素的影响.结论:上海市浦东新区三甲医院的多数出院老年患者存在医院-家庭过渡期护理服务需求.出院老年患者可接受的支付金额偏低,应进一步深入卫生经济学研究来为日后医院-家庭过渡期护理服务的收费提供标准.
Abstract
Objective:To investigate the demand and willingness to pay for hospital-home transitional care services among ol-der patients discharged from grade-A tertiary hospitals in Pudong New Area of Shanghai,so as to provide references for the pricing and reimbursement approaches of hospital-home transitional care services in the future.Methods:A cross-sectional survey utili-zing stratified random sampling techniques was conducted among 380 older patients discharged from grade-A tertiary hospitals in Pudong New Area of Shanghai.The demand for hospital-home transitional care services,willingness to pay,acceptable payment a-mount,and reimbursement rate were described statistically.The logistic regression was used to analyze the influencing factors of the acceptable payment amount.Results:There was a high demand for hospital-home transitional care services among discharged older patients.A majority(95.3%)expressed a willingness to pay,with an acceptable monthly payment amount of 251.89 yuan and 37.30 yuan per service.The desired reimbursement rates were 24.79%monthly and 33.75%per service.Factors affecting the accepta-ble payment amount included the status of chronic diseases,distance to medical resources,and the extent of family influence on medical decisions.Conclusion:The majority of discharged older patients exhibit the demand for hospital-home transitional care services in grade-A tertiary hospitals in Pudong New Area of Shanghai.The acceptable payment amount and desired reimbursement rate are relatively low,warranting in-depth health economics research to establish a pricing standard for hospital-home transitional care services in the future.
