广西紧密型县域医疗卫生共同体下牵头医院服务能力与运营效率评价

摘要
目的:对广西紧密型县域医疗卫生共同体(以下简称"县域医共体")下牵头医院的服务能力与运营效率进行评价,为深化紧密型县域医共体牵头医院高质量发展提供参考.方法:结合TOPSIS法与RSR法对广西县域医共体下12 家牵头医院服务能力进行评价,应用Malmquist指数对牵头医院运营效率进行评价.结果:2018-2021 年,牵头医院服务能力的Ci值依次为 0.1020、0.5490、0.6639 和 0.7767;2021 年,服务能力被评定为"优""中""差"的牵头医院分别为 2 家、8 家、2 家;2018-2019 年,牵头医院整体运营效率下降3.4%,有6 家牵头医院的全要素生产率变化值大于 1;2018-2021 年,牵头医院整体运营效率下降了 4.6%,有2 家牵头医院的全要素生产率变化值大于 1.结论:2018-2021 年,广西紧密型县域医共体下 12 家牵头医院服务能力总体呈上升趋势,但整体运营效率较低.政府部门要合理配置卫生资源,加快推进紧密型县域医共体建设;牵头医院要提高管理水平,引进高质量技术人才,提高资源利用效率.
Abstract
Objective:This study evaluates the service capability and operational efficiency of leading hospitals within the county-level medical consortia in Guangxi,aiming to provide insights for their further development.Methods:The service capabili-ties of the 12 leading hospitals were assessed using the TOPSIS and RSR methods,while the Malmquist index was employed to eval-uate their operational efficiency.Results:From 2018 to 2021,the Ci values indicating service capability of the leading hospitals were 0.1020,0.5490,0.6639,and 0.7767,respectively,showing a gradual increase over the years.In 2021,2 hospitals were ra-ted as"excellent",8 as"medium",and 2 as"poor"in terms of service capability.Between 2018 and 2019,there was a 3.4%de-crease in the overall operational efficiency of the leading hospitals,with 6 hospitals showing a total factor productivity change greater than 1.Over the entire period from 2018 to 2021,there was a 4.6%decrease in overall operational efficiency,with 2 hospitals dem-onstrating a total factor productivity change greater than 1.Conclusion:Between 2018 and 2021,the service capabilities of the 12 leading hospitals in Guangxi's integrated county-level medical consortia generally showed an upward trend,but their overall opera-tional efficiency was relatively low.Government departments should allocate health resources appropriately and accelerate the devel-opment of the integrated county medical community.Leading hospitals need to enhance management levels,attract high-quality technical talents,and improve resource utilization efficiency.
